Background The international development of health care, an ageing population and rapid technical development mean that more care is being performed in patient homes. This care environment is often unpredictable and involves both formal and informal caregivers, making it potentially unsafe. There is sparse knowledge about how patient safety is protected in home health care in Sweden and how registered nurses work to prevent risks and promote safe care. Aim The aim of the study was to explore registered nurses' efforts to reduce perceived risks for home‐dwelling older patients and ensure safe home health care. Method We used a qualitative design with individual interviews with 13 registered nurses working in municipalities in southeast Sweden. The narratives were analysed with inductive content analysis. Findings The findings showed that the registered nurses tried to secure a safe care environment and took an active role in care, striving to stay one step ahead of the patient. These three types of efforts are likely interdependent, suggesting they are all needed to reduce perceived risks for home‐dwelling older patients and ensure patient safety in home health care. Conclusions It is a challenge for registered nurses to maintain patient safety when performing care in patient homes. Continuity of care is required and must be based not only on self‐reliance among registered nurses but also on trusting relationships with patients, next of kin, colleagues and other personnel, as well as on the development of organisational conditions adapted to patient needs.

ABSTRAK Perawatan integratif dapat meningkatkan kualitas pelayanan keperawatan yang akan berkontribusi kepada peningkatan derajat kesehatan dan kualitas hidup pasien. Penerapan perawatan integratif membutuhkan kemampuan perawat dalam mengelola upaya preventif, promotif dan rehabilitatif secara efektif pada saat memberikan asuhan keperawatan pada area keperawatan medikal bedah (KMB). Pengabdian pada masyarakat ini dilakukan dengan tujuan untuk meningkatkan kapasitas perawat dalam perawatan integratif di area keperawatan medikal bedah. Metoda yang digunakan adalah in-house training berupa ceramah dan tanya jawab interaktif kepada 9 preseptor klinik dan 3 clinical case manager di RSUD Sumedang. Materi yang disampaikan diantaranya adalah Continuity of care pada chronic disease, Protocol follow up care pada area KMB, Transitional care, dan Discharge planning. Evaluasi kegiatan menggunakan analisis pre-posttest. Hasil kegiatan menunjukkan adanya peningkatan poin nilai rata-rata pre-post test (3,09). Nilai rata-rata pre-test sebesar 56 dan post-test sebesar 69,09. Dengan demikian, upaya yang sudah dilakukan bermanfaat dalam meningkatkan kapasitas perawat khususnya pengetahuan dan keterampilan menginisiasi integrative care. Upaya tersebut dapat diulang dengan target sasaran perawat medikal bedah lainnya selain perseptor maupun case manager. Kata Kunci: Keperawatan Medikal Bedah, Kapasitas Perawat, Perawatan Integratif, Preceptor, Case Manager, Pengabdian Masyarakat ABSTRACT Integrative Care could increase quality nursing services to be contribute to enhance the patient’s health status and quality life. Application of the integrative care requires the ability of nurses to effectively manage preventive, promotive and rehabilitative measurements when providing nursing care in the medical surgical nursing (MSN) area. This community service conducted with the purpose for increasing capacity of the nurses in integrative care in the MSN area. The method used was in -house training through interactive presentation and interactive discussion to nine clinical preceptors and three clinical case managers at Sumedang District Hospital. The topic presented included Continuity of care in chronic disease, Protocol follow-up care in the MSN area, Transitional care, and Discharge planning. The evaluation and outcome analysis using pre-post test design. The results of the program showed an increase in the average pre-post test score points (3.09). The average pre-test score was 56 and the post-test was 69.09. Thus, this community program that have been made are beneficial in increasing capacity nurse specifically knowledge and skills to initiate the integrative care . The future program is needed with other medical surgical nurses non preceptors and case managers. Stunting is still a nutritional problem for the community both at national and international levels. long and low birth weight history in is one of the risk factors of stunting. This research identifies whether there is an effect of continuity of care on length and birth weight. So that you can get a way to prevent stunting as early as possible. This research was conducted in Rejoyoso Village with a population of mothers who have babies aged 0-6 months, with a retrospective approach and the source of research data is the maternat book record. The research data were analyzed by chi square, and the result of birth length x2 count 5,605> x2 table 3,841 and the result of birth weight x2 count 5,029> x2 table 3,841. So the interpretation obtained from each variable is rejecting H0 or there is a continuity of care effect on length and birth weight
