A Confirmed Case of Toxic Shock Syndrome Associated with the Use of a Menstrual Cup

Abstract: Toxic shock syndrome (TSS) is caused by toxic bacteria and has been associated with prolonged, continual use of high-absorbency tampons. This article documents the first case of TSS associated with the use of a menstrual cup in a 37-year-old woman. The authors also discuss the history of TSS associated with tampon use and the mechanisms by which menstrual cups may also lead to the syndrome.

“…The presence of intravaginal or intrauterine devices has been described as a cause of fatal STSS [2]. The development of STTS in a patient using a menstrual cup for the first time has been described, with similarities to the present report [14]. In males, SA has been reported as an important cause of prostatic abscesses, some resulting in death [15].…”

“…A most important intervention in the management of TSS besides treatment mentioned so far is the administration of two agents one is clindamycin and intravenous immunoglobulin which is called adjuvant therapy 23 . Clindamycin has the advantage of halting the process of toxic antigen production thus inhibiting the perpetuation of shock state, tissue penetration and synergistic effect when used along with conventional antibiotic 24 . Clindamycin is bacteriostatic, recommended to be used only as an adjuvant agent with either penicillin or Cefazoline to avoid resistance 25 .…”

“…The mother expressed a legitimate doubt, which resonated with other participants of the FGD: there is indeed limited scientific research about RMMT that proves the safety of the cup for women's long-term health (Shihata & Brody, 2014). One study confirms one case of TSS associated with its use (Mitchell, Bisch, Arntfield, & Hosseini-Moghaddam, 2015) and another study reports a case of renal colic derived from its use (Nunes-Carneiro, Couto, & Cavadas, 2018).…”
