A Controlled Trial of a Prostacyclin and rt-PA in the Treatment of Severe Frostbite

Abstract: described in the published reports, 1,2 this calculation was erroneously made on the basis of the right eye only, resulting in fewer events as well as fewer people in whom a change in visual acuity could be ascertained. The published and revised numbers are presented in Table 1; the article is correct at NEJM.org. These changes do not materially change the conclusions of the study.

“…This may be due to lack of awareness of its use in frostbite treatment. More recent studies examining reperfusion of frostbite injuries have had some success with the use of local thrombolytics, 7,11,19 however, this was not observed in our study.…”

“…Severity of frostbite was categorized into superficial (1st and 2nd degree) or deep (3rd and 4th degree) ( Figure 1). 1,[10][11][12][13] Distribution of predisposing factors were compared between superficial and deep injuries using student t-tests for continuous variables and chi-squared or Fisher's exact tests for categorical variables. Patient encounters were reviewed to determine if the protocol was followed (Figure 2).…”

“…[1][2][3][4] The adage of "freezing in the winter and amputating in the spring" no longer holds. However, these advances in treatment have not gained widespread traction in northern Canadian centres, which for geographic reasons are likely to treat numerous patients with exposure to cold.…”

“…Administration of the vasodilator iloprost before initiation of alteplase is thought to have an additive effect, but this has yet to be demonstrated in trials. 1 In all studies using alteplase, heparin infusion has also been administered.…”

“…There are specific pathologies, for example, frostbite (Cauchy et al, 2011), suspension trauma (Mortimer, 2011), the triple H syndrome (hypothermia, hypercapnia, and hypoxia) , and high-altitude illness (Basnyat and Murdoch, 2003) that are not seen in other environments. Furthermore, trauma accounts for up to 90% of all cases in mountain rescue operations (Marsigny et al, 1999;McIntosh et al, 2010), and although the incidence of trauma is higher in urban areas, the relative mortality may be higher in rural cases (Fatovich et al, 2011).…”
