Background: Patients on hemodialysis (HD) commonly suffer from poor sleep quality which in turn compromises their quality of life and well as their mortality risk according to many studies. In Palestine however, no single study had emphasized the association and prevalence of sleep quality and daytime sleepiness among hemodialysis patients (HDP). Therefore, we conducted this study to determine the relationship between regular HD and the situation regarding sleep quality and daytime sleepiness in HDP. Methods: In this cross-sectional survey, we targeted HDP at one HD center in An-Najah National University Hospital. Sleep problems were assessed by both the Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS), which are considered as standardized tools for the assessment of sleep quality and daytime sleepiness, respectively. Results: Of the 192 dialysis patients who were interviewed, 167 were included in the final analysis of this study, with a response rate of 87%. The mean age of the participants was 57.6 ± 12.9. Fifty-two percent were males, and an equal percentage of the participants were 60 years old or older. The median ESS of the participants was 7 [4-11] and 43.1% of the participants had 9 or more ESS score, indicating excessive daytime sleepiness. The median PSQI score was 8 [6-12] and 76.65% of the participants scored more than 5 points on the PSQI, indicating poor sleep quality. Neither the ESS score nor the PSQI score were significantly associated with any particular category of participants, based on their demographic or clinical characteristics. Conclusions: In conclusion, we found that dialysis patients had poor sleep quality, evident by their high median PSQI score, though their excessive daytime sleepiness was less evident by their median ESS score in the high normal values. Additionally, we found a high prevalence of poor sleep quality among HDP, whereas excessive daytime sleepiness was less prevalent. Finally, no significant association was found between either of the two scores and the participants' demographic or clinical characteristics surveyed in this study.

Background: Chronic kidney disease (CKD) is one of the main public health issues. It increases the morbidity and mortality of patients. Treatment includes multiple aspects such as dialysis and lifestyle modifications. The primary goal of this study was to determine factors associated with self-efficacy and health-related quality of life (HRQoL) among haemodialysis (HD) patients. Methods: A cross-sectional descriptive correlation study was conducted on CKD patients undergoing HD at 12 different dialysis centres in Palestine. Self-efficacy was assessed by the Self-Efficacy for Managing Chronic Disease Six-Item Scale (SEMCD-6), and HRQoL was assessed using the Five-level EuroQol Five-Dimensions (EQ-5D-5L) tool. Multiple linear regression analysis was carried out to assess the association of factors with each of the SEMCD-6 and HRQoL scale scores. Results: A total of 283 HD patients were included in the study. A correlation test revealed moderately positive association between the EQ-5D and SEMCD-6 scores (r = 0.497, p value < 0.001). In multiple linear regression analysis, age, living status, and number of co-morbid diseases were negatively associated with SEMCD-6 scores (β = − 2.66, p = 0.016; β = − 5.71, p = 0.033; β = − 1.84, p = 0.006, respectively). Furthermore, there is a positive association between educational level and SEMCD-6 score with QoL score (β = 0.05, p = 0.017; β = 0.01, p < 0.001, respectively), while there is a negative association between the number of co-morbid diseases and QoL score (β = − 0.07, p = 0.001).Conclusions: This study assessed factors associated with impaired self-efficacy and HRQoL in HD patients. The results show that impaired self-efficacy was associated with the elderly, patients living with family, and patients with a high number of co-morbid diseases. Furthermore, this study found that the worst HRQoL was associated with patients with a low education level, lower levels of self-efficacy, and a high number of co-morbid diseases.

BACKGROUNDKidney transplantation is an expensive procedure, and it is an alternative treatment of end-stage renal disease other than dialysis. Cost-Effectiveness and the cost-utility study was undertaken at Sanglah Hospital Bali Indonesia, aiming to assess whether living-related kidney transplantation is more cost-effective and cost-utility than hemodialysis in the treatment of end-stage renal disease.METHODSA health technology assessment was done in living-related kidney transplant and hemodialysis during 2018. Data search from internet resources using the electronic library and critically appraised the best evidence of the data of the best data of five-year mortality and survival of kidney transplant and hemodialysis. A preliminary study about quality of life was also done among kidney transplant and hemodialysis patients. Data about kidney transplant costs and hemodialysis for five years was taken from the Sanglah General Hospital database. RESULTSDuring the study, Fourteen living-related kidney transplant patients at Sanglah Public Hospital Denpasar Bali Indonesia consisted of 14 recipients, 12 males, and 2 females aged 27 -55 years, and 14 donors, three males and 11 females aged 24-63 years were included. Thirty HEMODIALYSIS patients at the same hospital were recruited. They consisted of 20 males and ten females, with an average age of 50.9 years. The average five-year cost of kidney transplant was 741,078 M IDR. In comparison, hemodialysis's five-year cost was 581,514 M IDR, leading to a cost difference was 159,564 M IDR. Five-year mortality of kidney transplant was 55% (survival 45%), five years survival rate was 18.9%. Five years of survival difference between kidney transplant and hemodialysis was 26.1 %. Meanwhile, QOL for a kidney transplant was 0,7063, and QOL for hemodialysis was 0,5596, leading to a QOL difference of 0.15. Kidney transplantation will spend 6.11 M IDR for every 1% increase of survival for five years with 15% better QOL. In five years, kidney transplantation produces 67% ICER. From cost-utility perspective, kidney transplantation yields 31.04 M IDR per QALY or 2,217 USD per QALY.CONCLUSIONLiving-related kidney transplantation is more cost-effective and cost-utility than hemodialysis in the treatment of end-stage renal disease.
