India's commitment to achieve the goal of Universal Health Coverage is evidenced by the launch of Ayushman Bharat and the transformation of Sub-Centres into Health and Wellness Centre to provide secondary and comprehensive primary healthcare to the vast majority of its population. Successful implementation of these initiatives requires adequate and skilled human resources for health and a conducive work environment. There exists a deficiency of doctors and paramedical professionals in different parts of the country. The vacancies in different categories of health functionaries have increased after 2005 despite the National Rural Health Mission/National Health Mission initiatives to strengthen the health system. The distribution of doctors and paraprofessionals in health is skewed, favoring urban areas. Properly oriented, trained, and skilled health workforce and informed public participation are critical to provide quality services for achieving national health goals. Therefore, it is necessary to establish public health cadre in all states of India and ensure appropriately skilled workforce to meet the functional requirements of health-care delivery system at different levels. This will also help to move forward on the way to reach the Sustainable Development Goals.

The Postgraduate Diploma in Public Health Management (PGDPHM) was launched in 2008 to create a cadre of public health managers within the ranks of the public healthcare system. However, limited research exists on this programme and its impact on the health system. This qualitative research study aimed at obtaining in-depth perspectives of PGDPHM graduates from the Indian Institute of Public Health-Gandhinagar (IIPHG). Study participants were selected from a variety of roles across Gujarat. The programme equips healthcare professionals with several competencies essential to the management of public health issues from the community to state levels. However, the health system has yet to fully realise its potential and rationally allocate this human resource. Recognition also has not translated into public health oriented career growth for most graduates, and the duties of medical officers with this qualification continue to have a clinical focus. The PGDPHM programme has the potential to help ensure that India reaches an adequate capacity for health management professionals. It would be beneficial to improve recognition for the programme within the health system, increase focus on public health research and conduct regular evaluations to assess the career trajectories of graduates.

The relevance of public health has been emphasized in the wake of the global epidemic COVID-19. There are several success stories that we often tend to forget, such as the fight against various infectious illnesses like smallpox, poliomyelitis, and current human-immunodeficiency virus (HIV) prevention, to name a few, public health has played a significant impact. Diarrheal diseases, for example, which contributes significantly to India's under-five mortality rate and is one of the leading causes of malnutrition, can be effectively handled by improving access to safe water and sanitation. Because public health encompasses more than just health, we require a workforce with managerial and leadership skills as well as training in public health as a specialty. This paper explores some of the successes and lessons learned from systematic investments in public health in the Indian state of Tamil Nadu, namely The Tamil Nadu model and other countries, as well as the system's flaws. In India, a feasible framework for establishing dedicated public health cadres has also been explored. Evidence was acquired from PubMed, Google Scholar, newspaper stories, and publicly released government orders and papers. The recruitment of cadres may resemble that of the Indian economic/statistical services (IES/ ISS) by the UPSC. Another area to emphasize for health professionals interested in public health is training. Starting from frontline workers, block level workers to district and state we need dedicated public health workforce. Moreover, the need of the hour is to establish such a system which will work alongside pre-existing clinical fields.
