Embedded WAIS-IV Detection Strategies and Feigned Cognitive Impairment: An Investigation of Malingered ADHD

Malingered attention-deficit hyperactivity disorder (ADHD) may be strongly motivated on college campuses by recreational use of ADHD medications and to obtain unwarranted academic accommodations. Rather than rely on face-valid (easily faked) ADHD checklists, the study focused on the more complex Wechsler Adult Intelligence Scale–Fourth edition (WAIS-IV; Wechsler, 2008). However, the current literature has not yet investigated well-defined detection strategies for feigned WAIS-IV presentations. Using aprioristic standards, four different detection strategies from the feigning literature were adapted to certain WAIS-IV subscales. For example, significantly below-chance performance was applied to visual puzzles. Using a between-subjects simulation design, 74 undergraduate simulators were compared with archival data on 73 outpatients diagnosed with ADHD at a university psychology clinic. Very large effect sizes (Cohen’s ds from 1.66 to 1.90) differentiated between genuine and feigned ADHD. Two strategies (significantly below-chance performance and floor effect) showed strong promise if cross-validated for other feigning presentations. The study concluded with clinical considerations and future avenues for research.

Reference [1] presented a skewed perspective of the M-FAST literature base and provided the flawed conclusion that the M-FAST should no longer be used in practice. In an attempt to correct the many issues with [1], this article provides a narrative review of the strengths and weaknesses of research findings for the M-FAST interpretation as well as reviews methodological concepts underlying feigning research. The M-FAST was designed to screen for potential feigning of psychiatric symptoms. It was not designed to conclude that an examinee is feigning or malingering psychiatric symptoms. A positive result on the M-FAST only indicates that additional data needs to be collected to make the aforementioned conclusions. Applying the M-FAST in any other way is a serious error on the part of the user. The research literature thus far generally supports the use of the M-FAST cut-off as a screening measure for possible feigning of psychiatric symptoms. However, there are scenarios when the M-FAST may not operate as efficiently, and these scenarios are discussed. Reference [1] misrepresented the purpose of the M-FAST as well as research findings on the M-FAST. Therefore, [1] should be read with great caution.

Psychologists who work as therapists or administrators, or who engage in forensic practice in criminal justice settings, find it daunting to transition into practice in civil cases involving personal injury, namely psychological injury from the psychological perspective. In civil cases, psychological injury arises from allegedly deliberate or negligent acts of the defendant(s) that the plaintiff contends caused psychological conditions to appear. These alleged acts are disputed in courts and other tribunals. Conditions considered in psychological injury cases include posttraumatic stress disorder, depression, chronic pain conditions, and sequelae of traumatic brain injury. This article outlines a detailed case sequence from referral through the end of expert testimony to guide the practitioner to work effectively in this field of practice. It addresses the rules and regulations that govern admissibility of expert evidence in court. The article provides ethical and professional guidance throughout, including best practices in assessment and testing, and emphasizes evidence-based forensic practice.
