A double‐blind, controlled, crossover trial of amlodipine on iron overload status in transfusion dependent β‐thalassemia patients

Abstract: Background and aim: This study examined whether administration of amlodipine could improve myocardial iron loading status in patients with transfusion dependent β-thalassemia (TDT), through a placebo-controlled, crossover study. Methods: Amlodipine (5 mg, daily) or placebo were prescribed to all patients (n = 19) for 6 months, and after a 2-week washout period, patients were crossed over to the other group. The efficacy of amlodipine on iron loading was assessed by measuring myocardial T2*-weighted magnetic re… Show more

“…All outcomes were evaluated at 6 and 12 months, depending on which was reported in the study. The study (Karami et al 16 ) was a cross‐over study with two groups: The AP sequence and the PA sequence. In the AP sequence, amlodipine was used for 6 months, followed by a placebo for another 6 months.…”

“…In psoriasis, it has been shown that external antigen triggers innate immunity and activates plasmacytoid dendritic cells, which produce IL-23 and TNF-alpha to differentiate naive T cells into Th17 cells. Then Th17 cells produce IL-17 to promote epidermal proliferation [1][2][3]. The basal cell responds to IL-17 stimuli for proliferation and differentiation to produce a thickened epidermis.…”

“…Psoriasis is one of the most common chronic immune-mediated skin diseases with an estimated global prevalence of 2-3% 1 - 3 . It is primarily a disease in which the skin barrier is compromised due to the destabilization of cell proliferation and differentiation 4 , 5 .…”

“…Amlodipine is a dihydropyridine-type calcium blocker that may impede calcium influx. Amlodipine is available at a lower price, with oral administration, proven safety, and a long half-life (35 to 50 h) [5,17]. Some clinical trials have shown the significant impacts of amlodipine on reducing iron overload.…”

“…Some clinical trials have shown the significant impacts of amlodipine on reducing iron overload. The lack of sample size in the prior studies on β-thalassemia cases necessitates conducting a review on this issue [4,5,17,18].…”
