A double-blind, placebo-controlled, dose–response study of the effectiveness and safety of lisinopril for children with hypertension

Abstract: Lisinopril, once daily, is an effective and well-tolerated antihypertensive in children aged 6 to 16 years. An initial dose of 0.07 mg/kg, administered once daily, effectively lowered BP within 2 weeks. Blood pressure was reduced in a dose-dependent fashion.

“…No long-term randomised trials have investigated the effect of kidney function and cardiovascular disease in children or adolescents with diabetes, and the consensus statements are based on extrapolations from adult studies. In short-term studies, ACE inhibitors have been effective and safe in non-diabetic hypertensive children [58,59], but long-term studies are lacking.…”

“…Adolescenti, osobito oni s lošom kontrolom ŠB-a mogu imati brzu progresiju retinopatije 55 . Najsenzitivnije metode za detekciju retinopatije su stereoskopsko fotografiranje očnog dna i fluorescinska angiografija 56 . Retinopatija započinje mikroaneurizmama, hemoragijom i eksudatima.…”

“…U pacijenata s perzistentnom mikroalbuminurijom preporučuje se terapija inhibitorima angiotenzin konvertirajućeg enzima (engl. angiotensin converting enzyme; ACE) s ciljem zaustavljanja progresije daljnjeg bubrežnog oštećenja 56 . U djece i adolescenata oboljelih od ŠB-a tipa 1 neuropatija se rijetko vidi.…”

“…However, its three times daily dosing is inconvenient for patients and families, so oncedaily agents such as lisinopril and enalapril are often used instead, though they have been studied less than captopril in children. Lisinopril has been studied in a double-blind placebo-controlled pediatric study and generally costs less than newer ACE inhibitors [41]. Enalapril has also been studied in children and is currently the only commerciallyavailable solution, though at this time the commercial solution may be more costly than compounded lisinopril, for example.…”
