A false-positive finding in therapeutic evaluation: hypermetabolic axillary lymph node in a lymphoma patient following FDG extravasation

Abstract: We report a case of a false-positive finding in FDG PET/CT following radiotracer extravasation. A 15-year-old male patient was referred for therapeutic evaluation status post-chemotherapy for a lymphoblastic lymphoma. FDG PET/CT showed discordant findings with a marked decrease in a liver/hepatic hilum uptake, disappearance of a subcutaneous left supraclavicular uptake, and appearance of intense right axillary nodal uptake. Extravasation in the right superior limb was noted. Comparison with the previous scan s… Show more

“…18 ). Regardless of intense FDG uptake, each node seems to be normal in size and has a fatty hilum [ 82 ]. A marked tracer distribution within the subcutaneous tissue of the upper limb around the injection site indicates tracer extravasation, even if the patients or the medical staff who administered the tracer have not been aware of it.…”

“…In the same way, axillary lymph nodes can be a source of false-positive results. A usual case is tracer extravasation [29,30]: the lymph node is usually not enlarged and shows an adipose tissue component. In clinical practice, axillary lymph node uptake after treatment should be considered as pathologic only if associated with another abnormal uptake, increasing the rate of true positivity from 20.8 to 79.6%, [31].…”
