A False-Positive Localization of a Parathyroid Adenoma With Technetium Tc 99m Sestamibi Scan Secondary to a Thyroid Follicular Carcinoma

Abstract: Using technetium Tc 99m sestamibi for localization of a parathyroid adenoma in primary hyperparathyroidism, we describe the first reported case of increased uptake due to a thyroid follicular carcinoma. Prior false-positive results have been reported exclusively with follicular adenomas.

“…A 280-mg right superior and an 820-mg right inferior parathyroid adenoma were surgically resected. primary thyroid lymphomas, a cervical lymph node metastasis from papillary thyroid carcinoma, reactive lymph nodes, a remnant thymus, a PTH-secreting paraganglioma, and an enlarged submandibular salivary gland (60)(61)(62)(63)(64)(65)(66)(67). However, in the clinical setting of hyperparathyroidism, false-positive findings are uncommon.…”

“…It is widely used for tumoral imaging where FDG-PET is not available. Among the patient group with increased thyroglobulin levels but negative There are several causes that may lead to misinterpretation of TMS for thyroid carcinoma like central venous pacemaker wires and parathyroid adenomas 8,9 , but our patient had normal parathyroid hormone and calcium levels and had no intravascular instrumentation. Her thoracal MRI was interpreted as normal in mediastinum, and showed no abnormal structure which may correspond to the uptake in TS and TMS.…”

“…A thyroid adenoma or multinodular goiter may exhibit abnormal retention of sestamibi, thus mimicking a parathyroid adenoma. 15 Both of our false‐positive findings were thyroid adenomas that happened to lie on top of the parathyroid adenomas. The probe can be helpful in this situation because it shows the depth of the abnormal gland.…”
