A feasibility study of home telemedicine for patients with cystic fibrosis awaiting transplantation

Abstract: Sixteen patients with cystic fibrosis were randomized to either telemedicine or standard care alone. All patients were terminally ill. Patients in the telemedicine group had an ISDN line installed in their home and were given a videoconferencing unit connected to their home television set. Eleven subjects completed the baseline assessment and seven patients completed the study (4 on telemedicine and 3 in the control arm). Telemedicine patients had weekly videoconferences from home for a clinical assessment, ps… Show more

“…Within the home, smartphones can enable self-reporting of symptoms of respiratory exacerbation, to reduce delay to treatment [66,67]. The feasibility of video conferencing alongside remote data collection via 'nonconnected' devices has also been explored [65], as have home-based lung function tests supported by regular telephone engagement with the clinical team [59]. These approaches have been shown to support improved adherence to treatment amongst patients [58].…”

“…Tele-hospice was able to decrease spiritual and medical burden, improving quality of life and symptom management for veterans and families coping with end of life [114]. Home tele-health consultations are a feasible and acceptable means of facilitating a palliative care consultation reducing the burden on families also in paediatric patients [115,116].…”

“…We strongly advocate that centres have regular feedback about their patients to allow them to monitor and review them from afar, and identify new equipment problems. With the development of new technology [13] and improved telemedicine options [14,15], even in very vulnerable patients [16], it is possible to download symptom scores, arterial oxygen saturation levels and ventilator performance details on tidal volume, pressure and leaks daily [14,15]. Indeed, there are pilot studies in progress with some ventilators that are able to monitor pulmonary mechanics and adapt settings accordingly.…”
