A Flexible Cystoscope

Abstract: An assessment of the prototype Olympus Flexible Cystoscope in routine urological practice is presented. The results show that the instrument compares favourably in all respects with standard rigid equipment and has the advantage that cystoscopy can be performed supine and under local anaesthesia with ease. Biopsies may be taken; small lesions fulgurised and ureteric catheterisation can be performed without general anaesthesia.

“…Previous work outlining the value of out-patient flexible cystoscopy has confirmed its diagnostic accuracy, cost-efficiency and therapeutic value (when used in conjunction with a YAG laser for the treatment of small bladder tumours). Some reviews have hinted at a high level of patient tolerance (Clayman et af., 1984;Fowler, 1984;Fowler et a/., 1984;Matthews et al, 1984;Powell et al, 1984;Soloway, 1985). However, one of the major criticisms of the technique is its potential physical and emotional invasiveness for the conscious patient.…”

“…Intrinsic to this change in procedure is the need for patients to tolerate more discomfort and indignity associated with urogenital examination and instrumentation while fully awake and alert. Although several reports on the value of flexible cystoscopy have paid passing attention to the patients' feeling about the procedure (Clayman et al, 1984;Fowler, 1984;Fowler et al, 1984; Read at the 44th Annual Meeting of the British Association of Urological Surgeons in Buxton, June 1988 Matthews et al, 1984;Powell et,al., 1984;Soloway, 1989, no accurate quantitative assessment of patient tolerance has been made. It is important that the necessary improvements in efficiency in surgical practice are not made at the expense of patients' suffering.…”

“…39 This technique of examination has now been utilized by urologists to perform office cystoscopy, bedside cystoscopy in critically ill patients, retrograde pyelography, biopsy and fulguration of tumors, placement of ureteral stents with fluoroscopic guidance, and urodynamic studies. [40][41][42][43] Loughlin and Yalla in 1986 40 combined flexible cystoscopy and urodynamics, becoming the first to describe endourodynamics. They recorded urethral pressure profiles (UPP) using a flexible cystoscope to aid in determining the urethral location of the pressure sensor during micturition.…”

“…Patients were then referred the same day to the Out-patient Flexible Cystoscopy Clinic where they had a cystoscopy under local anaesthesia. The technique used for this investigation was similar to that described by Fowler [6] and Powell et al [7].…”
