A Gigantic Anogenital Lesion: Buschke-Lowenstein Tumor

Abstract: Buschke-Lowenstein tumor is a relatively rare sexually transmitted disease. It is a neoplasm of the anogenital region which has benign appearance on histopathology but is locally destructive. It carries a high recurrence rate and a significant potential for malignant transformation. Human papilloma virus has been implicated as an etiologic agent for this tumor. Since this disease is rare and no controlled studies exist, radical excision of this anogenital lesion is generally recommended as the first line thera… Show more

“…According to the literature, neoadjuvant chemoradiotherapy is considered necessary only in cases of high risk of malignancy and Safi et al confirm that neoadjuvant radiotherapy shows a higher incidence of free margins in cases of anal sphincter infiltration (9). On the other hand, a higher risk of anaplastic cancer transformation after radiotherapy is reported by others (10,11).…”

“…For the histological diagnosis, large and deeper biopsy should be performed to ensure that no malignant cytological characteristics are missed in supericially biopsied specimens. Microscopic features are thick stratum corneum, marked papillary proliferation, tendency to deep invasion, with displacement of surrounding tissues, negligible cellular atypia and a low mitotic rate [25,28]. Similar features are also seen in verrucous carcinoma.…”

“…Isso faz com que a excisão cirúrgica completa seja o tratamento de escolha, 7,9,11,12 ressaltando-se que a radioterapia e a quimioterapia também podem ser empregadas para evitar que o cancro se estenda para o canal anal. Em um estudo de seguimento realizado por 21 anos em 24 pacientes diagnosticados com CEC anal e tratados, 8 pacientes foram tratados com ampla excisão local e nesses não foram detectadas nem recorrência, nem mortalidade, dado consistente com a literatura.…”
