A glimpse of hope: cardiac surgery in low- and middle-income countries (LMICs)

Abstract: Currently, more than five times more people live in low-and middle-income countries (LMICs) than in high-income countries (HICs). As such, the downward trend in cardiac surgical needs in HICs reflects only the situation of one sixth of the world population while the vast majority living in LMICs has still no or limited access to life saving heart operations. In these countries, rheumatic heart disease (RHD) still accounts for a significant proportion of cardiac surgical needs. In low-and lower-middle income co… Show more

“…Although annual needs for replacement valves by far exceed a million patients (64,65) and >3 million patients are in need of dialysis access, the highest number of patients potentially benefitting form a tissue engineering approach would still be those with congestive chronic heart failure (CHF). Over the decades, the number of patients with chronic heart failure has exponentially increased due to prolonged life expectancies and the progressive nature of cardiovascular diseases (66,67).…”

“…As stated in our joint assessment of "Global unmet needs in cardiac surgery" (3) Oceania, RHD outweighs other indications including congenital heart defects by 3-4:1. (2,4) To rather conclude that "Expensive valve replacement surgery in Africa is a disaster and should not be advocated" is out of…”

“…RHD remains the single most common cardiovascular disease in young adult and adolescent patients in need of heart surgery, outweighing other indications such as congenital cardiac defects by almost 4-fold [ 3 , 4 ] . The mean age of patients submitted to surgery is around 20-25 years, half of whom require operation under the age of 20, some even under the age of 10 [ 5 ] .…”
