A Hand‐Held Ultrasound Device for the Assessment of Peripheral Nerves in Leprosy

Abstract: BACKGROUND AND PURPOSE Mycobacterium leprae complex affects peripheral nerves, causing nerve enlargement. This proof‐of‐concept pilot study was conducted to determine if a small hand‐held ultrasound device can accurately identify nerve involvement in individuals with leprosy. METHODS Peripheral nerve ultrasound was conducted of the bilateral median (wrist, forearm, elbow, and mid‐humerus), ulnar (wrist, forearm, elbow, 4 cm proximal to the elbow, and mid‐humerus), C5 root, and greater auricular nerves with a s… Show more

“…The sensitivity of conventional PCR (17 studies) was 75.3% (95% CI 67.9-81.5) and specificity was 94.5% (95% CI: 91.4-96.5). Regarding peripheral nerve ultrasonography in the evaluation of HD neuropathy, several studies have considered it as a method with good efficacy, reproducibility, and diagnostic accuracy, besides its accessible cost and availability, even for point of care (40,43,47,49,59,(69)(70)(71)(72)(73)(74)(75)(76).…”

“…It provides an objective measure of the nerve dimensions in addition to revealing structural changes over a longer length of the nerve ( 49 ). Enlarged nerves of patients undergoing leprosy reactions show increased endoneurial and epineurial vascularity that can be measured by ultrasonography with color or power Doppler ( 24 , 28 ). We also used ultrasonography of the posterior tibial nerve to show that armadillos, like humans, show enlargement of peripheral nerves due to M. leprae infection.…”

“…Standardized tests that measure nerve damage are well established for humans (23) and there has been some use of these tests to evaluate neuropathy in persons affected by leprosy (2,(24)(25)(26)(27)(28). Validation of such tests in M. leprae-infected armadillos would support its use as an animal model to study leprosy NFI.…”

“…With technological advances, smaller ultrasound machines are available which can be used at the point-of-care. 10 Paramedical field workers can probably be trained to identify thickened nerves more reliably by HRUS than by clinical examination; besides the ultra-sound data can be verified subsequently by an expert or in real-time using tele-medicine services.…”
