A Latent Class Approach to Modeling Trajectories of Health Care Cost in Pediatric Cystic Fibrosis

Abstract: Introduction. Estimating costs of medical care attributable to treatments over time is difficult due to costs that cannot be explained solely by observed risk factors. Unobserved risk factors cannot be accounted for using standard econometric techniques, potentially leading to imprecise prediction. The goal of this work is to describe methodology to account for latent variables in the prediction of longitudinal costs. Methods. Latent class growth mixture models (LCGMMs) predict class membership using observed … Show more

“…Individual‐level variation in clinical characteristics, in response to treatment and in the use of services can be incorporated in patient‐level microsimulation (health‐state transition or discrete event simulation) (Ramaekers et al, 2013). Latent class models of various types have already been used to estimate distinct trajectories of service use and costs (Levy & Rosenberg, 2019; Mau et al, 2020; Schneider et al, 2020). Such analyses require longitudinal repeat assessment cohort studies with sufficient depth to capture outcomes and service use across individuals.…”

“…This approach analyzes longitudinal data by fitting a semiparametric (discrete) mixture model, estimating each individual's probability of membership in each group, and assigning them to the group based on their highest membership probability. We chose to use this type of modeling approach because it allows for the specification of latent classes and can allow for different functional forms and because prior work has suggested it leads to less complex models (32,33). We modeled the longitudinal cost trajectories using calendar month as the time variable, costs in each month, order = 4 (i.e., 4-degree polynomial), and a censored-normal distribution (17,29,31).…”
