A meta‐analysis of MitraClip combined with medical therapy vs. medical therapy alone for treatment of mitral regurgitation in heart failure patients

Abstract: Compared with conservative treatment, MitraClip is associated with a significant survival benefit. Importantly, this superiority is particularly pronounced among patients with functional MR and across all the main subgroups.

“…To our knowledge this is the first metaanalysis comparing MitraClip along with medical therapy versus medical therapy alone selectively in FMR patients only. Previous meta-analyses performed on the same topic have included either observational studies or included patients with degenerative mitral regurgitation (DMR) in their analysis [15,16]. Given the pathophysiology of DMR patients is fundamentally distinct from that of FMR patients, the inclusion of DMR patients along with FMR patients in these meta-analyses may potentially act as a confounding factor and may, therefore, skew the overall results in either direction.…”

“…While there are some conflicting reports, interventional percutaneous edge-to-edge repair of the mitral valve (MitraClip) has been shown in a number of studies to improve quality of life, left ventricular volumes and possibly reduce mortality, and so should be considered in those with severe secondary mitral regurgitation. 32,33 There are less data to support surgical repair for mitral regurgitation. 34…”

“…These results indicate a major advancement in the treatment of heart failure. A recent meta-analysis including 2121 heart failure patients confirmed the findings of COAPT by demonstrating a significant survival benefit in those treated with the MitraClip device [4]. 692 Transcatheter mitral valve repair using the MitraClip: which patients benefit most?…”
