A Modified Surgical Technique for Minimally Invasive Arthroscopic Total Wrist Fusion

Abstract: Background Arthroscopic total wrist arthrodesis is a procedure with minimal violation of extensor compartments and less scar formation, compared with open wrist arthrodesis. In this paper, we would like to present our experience with a modified surgical technique for arthroscopic total wrist arthrodesis. Description of Technique After an arthroscopic cartilage removal through radiocarpal and midcarpal portals, the operation is continued under fluoroscopic control. In this technique, four wires are … Show more

“…The RSN usually bifurcates around 4.2 cm proximal to Lister tubercle, and the dorsal branch bifurcates again at about the level of Lister tubercle. 7,9,17 To be the most clinically relevant, we evaluated distances from portals to the closest RSN branch. For the 1,2 portal, this was either the most radial or the middle branch of the SBRN.…”

“…More complex conditions, such as perilunate dislocations, could be treated in a minimally invasive manner by arthroscopic-assisted procedures as well 20 . Another example is the quite aggressive open partial or total wrist arthrodesis procedures, which now could also be performed arthroscopically 9,21 . Thirdly, the use of wrist arthroscopy could aid the diagnosis of many complex wrist conditions 8 .…”

“…72 While the standard procedure is performed openly, it can also be performed through arthroscopy with less damage to the extensor compartments and a better esthetic outcome. According to the technique by Nazerani et al, 73 the cartilage can be debrided through the radiocarpal and midcarpal portals, and the joints can be fixed using four self-tapping cannulated screws percutaneously under fluoroscopic control.…”
