A Müllerian Prostatic Cyst Protruding into the Base of the Urinary Bladder

Abstract: The unique appearance of a midline prostatic cyst is reported. The cyst presented as a round-shaped filling defect at the base of the urinary bladder during an intravenous pyelography study. Transabdominal sonography showed the mass to be a midline prostatic cyst. The sonographic features of the mass and its location are the hallmark of a müllerian duct prostatic cyst. Midline prostatic cysts are discussed and the features differentiating müllerian duct cysts from utricle cysts are emphasized.

“…Sperm is never found in the mullerian duct cyst [10]. Besides, a prostatic utricle cyst is usually restricted to the prostate region, while the mullerian duct cyst may extend over the base of the prostate and form a visible protrusion into the bladder [11]. It should be noticed that not all cystic lesions located at the midline of the prostate are müllerian duct cysts or prostatic utricle cysts.…”

“…Anatomically, these cysts derive from the caudal ends of the fused Mullerian duct, originating at the level of the verumontanum, entering medioposteriorly [5]. As the cyst grows to extend superiorly to the prostate gland, it may press on the proximal prostatic urethra, ejaculatory duct, bladder trigone or rectum [6]. The content of the Mullerian duct cyst has been variously described as serous, mucoid, purulent or hemorrhagic.…”
