A Multidisciplinary Approach for Management of Postenucleation Socket Syndrome with Dermis‐Fat Graft and Ocular Prosthesis: A Clinical Report

Abstract: A contracted eye socket is a cosmetic blemish to the patient. It not only renders patients unable to wear an eye prosthesis, but also becomes a source of chronic discharge and irritation. Orbital implants allow for cosmesis and volume replacement of an enucleated or eviscerated eye. Alloplastic orbital implants are associated with potential complications, including exposure and extrusion. A dermis-fat graft offers the advantages of relative availability and an autologous nature. “…Depending upon the extent of socket contraction, the surgeon has several surgical procedures at his or her disposal . For mild socket contraction, autogenous dermal fat grafting and fornix deepening sutures have been advocated . For moderate socket contraction, a mucous membrane graft may be employed .…”

“…After 6 months, the patient was fitted with a new, custom‐made ocular prosthesis (Fig ) . As in Aggarwal et al, the procedure was initiated by fabricating a custom impression tray, using autopolymerizing polymethyl methacrylate (PMMA) resin (Trevalon; Dentsply India Pvt. Ltd., Gurgaon, India) in its dough stage and molding it by adapting to form an ocular‐shaped tray .…”

“…In (50%) of the patients in the study group, custom-fit prosthesis were placed after surgical procedures like evisceration and enucleation. Additional surgical procedures like Fornix deepening sutures, Dermis-fat grafts 4 and orbit reconstruction was required in 30 cases accounting for 15% of the study group.…”
