A Multilead Scheme Based on Periodic Component Analysis for T-Wave Alternans Analysis in the ECG

Abstract: Abstract-T-wave alternans (TWA) is a cardiac phenomenon that appears in the electrocardiogram (ECG) and is associated with the mechanisms leading to sudden cardiac death (SCD). In this study, we propose the use of a multilead TWA analysis scheme that combines the Laplacian likelihood ratio (LLR) method and periodic component analysis (pCA), an eigenvalue decomposition technique whose aim is to extract the most periodic sources of the signal. The proposed scheme is evaluated in different scenarios-from syntheti… Show more

“…This TWA detector has also been tested and successfully applied on invasive EGM signals [103]. Although all these methods are usually applied on a leadby-lead basis, works using a multi-lead strategy suggest that improved performance can be achieved by jointly processing all the available leads, taking advantage of the different interlead correlation of TWA and noise components [104].…”

“…We present here two multi-lead approaches [104], one based on πCA (multi-πCA) [105] and another one based on PCA (multi-PCA) [89]. Both approaches follow a general scheme whose main stages are: preprocessing, signal transformation, TWA detection, signal reconstruction and TWA estimation (Fig.…”

“…For estimating TWA amplitude of each segment (V k ), we replicated the analysis performed in [2]: the three orthogonal leads were linearly combined in order to maximize the TWA content over noise by using πCA [6] and then, the LLRM [7] was applied in the new combined lead to estimate the TWA waveform of each segment, expressed as the median difference between ST-T complexes of even and odd beats.…”

“…The SM has been used to measure TWA in some specific clinical procedures such as stress tests and, in any case, analysis is restricted on heart rates comprised between 100 and 125 bpm [9] while the MMAM has been applied to Holter recordings, requiring visual inspection of the segments with TWA. In 2010 a new multilead index based on the Laplacian Likelihood Ratio method combined with Periodic Component Analysis, was developed [10] which detected TWA by maximizing the ECG components at the TWA frequency, i.e., 0.5 cycles per beat (cpb). In a second study [11] the method was used to measure TWA on a database of 650 Holter ECG recordings (MUSIC [12]).…”
