A multimodal regional intervention strategy framed as friendly competition to improve hand hygiene compliance

Abstract: ObjectiveTo investigate the effects of friendly competition on hand hygiene compliance as part of a multimodal intervention program.DesignProspective observational study in which the primary outcome was hand hygiene compliance. Differences were analyzed using the Pearson χ2 test. Odds ratios (ORs) with 95% confidence interval were calculated using multilevel logistic regression.SettingObservations were performed in 9 public hospitals and 1 rehabilitation center in Rotterdam, Netherlands.ParticipantsFrom 2014 t… Show more

“…26,36,37 Our experience with this program reinforced the findings of other studies that a multimodal, team-driven, friendly competition and positive-reinforcement-focused approach is crucial for improving hand hygiene, particularly with respect to peer influence and role modeling. 11,12,28,29,38 It also reflects the assertion outlined in other studies that multimodal interventions are necessary for sustained improvement. 26,39,40 Easing concerns about the system's accuracy and increasing comfort were important considerations.…”

“…26 Interventions that are associated with improved adherence include providing feedback, rewarding positive behavior, encouraging friendly competition, and conducting educational programs to raise awareness. [26][27][28][29][30] Our institution implemented an RFID-based AHHMS in selected units, coupled with interventions designed to increase hand hygiene adherence. We assessed hand hygiene adherence both electronically and by direct observation, and we studied the effect of AHHMS-measured adherence on HAI rates.…”

“…It is suggested that effective measures should be further implemented to improve compliance rate of caregivers. HH compliance also differed between ward type, with the pediatrics ward in the study showing the highest compliancy of 60%, whereby this nding is higher than other studies in the literature [18,26,29] . Since we made fewer observations in the pediatrics ward, there is a need for more detailed investigation of pediatrics ward so that this result can be con rmed.…”

“…This is often attributed to diagnosed MDRO inpatients and caregivers receiving health education about HH and having access to adequate supplies of equipment like handwashing materials [26,27] . Optimal levels of adherence were not achieved, however, since HH should be performed in at least 80% of case to interrupt cross transmission in settings with high infection risks [28,29] . It is suggested that effective measures should be further implemented to improve compliance rate of caregivers.…”

“…This may motivate top management as a key performance indicator for the department. In a friendly competition setting, a multimodal intervention program significantly increases compliance with HH [ 33 ].…”
