A national survey of prevention of infection in obstetric central neuraxial blockade in the UK

Abstract: SummaryWe conducted a postal survey of all consultant-led UK obstetric anaesthetic units in August 2009, to assess the standard of aseptic technique used for neuraxial blocks. One hundred and sixty-four units responded giving a response rate of 76%; 93% of units (149 ⁄ 160) follow recommended precautions and attach a bacterial micropore filter to the epidural catheter. Epidural top-ups are provided by 72% (116 ⁄ 162) of units, with about two thirds using premixed solutions (of local anaesthetic with opioid) in… Show more

“…45,46 No systematic review has evaluated these recommendations, and wide variations in what is considered to be essential aseptic precautions for placement of epidural catheters for labor and delivery have been documented. 46,47 Pregnant women are at increased risk of influenza-related morbidity and mortality, seasonal and pandemic strains alike. 48,49 Persisting risk of severe influenza-related illness was observed for at least two weeks postpartum during the 2009 H1N1pandemic influenza A season, prompting CDC to extend its recommendation for treatment with antiviral medications for the first two weeks postpartum.…”

“…Changes in epidural pressure related to breathing, coughing, or straining in labour may in turn result in significant movement of epidural fluid within the catheter [6], and in vitro studies suggest that fluid commonly drains proximally towards the epidural space following accidental disconnection [17]. Thus, it is prudent to consider all disconnected epidural catheters as potentially contaminated, and although a range of opinion exists on the optimal management of accidental disconnection [19, 20], in our institution resiting of the epidural is recommended in virtually all cases.…”

“…Numerous studies demonstrate useful synergy between the two research traditions, including ‘mixed‐methods’ studies comprising both qualitative and quantitative study of the same problem or phenomena , qualitative work that is analysed with statistical methods , ‘cohort’ studies that have quantitative outcomes but are observational in nature , surveys and proforma‐based studies combining forced‐choice and free‐text data , and experimental studies that collect data using qualitative methods . That the authors of such work have chosen to combine elements of qualitative and quantitative research to address complex questions is further evidence that the boundaries between qualitative and quantitative research are less distinct than they may at first appear.…”
