IntroductionCarbapenem and colistin-resistant Enterobacteriaceae, including Klebsiella pneumoniae, have become a growing global concern, posing a significant threat to public health. Currently, there is limited information about the genetic background of carbapenem and colistin-resistant K. pneumoniae isolates infecting humans and dogs in Thailand. This study aimed to characterize carbapenem and colistin-resistant genes in six resistant K. pneumoniae clinical isolates (three from humans and three from dogs) which differed in their pulse field gel electrophoresis profiles.MethodsMatrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS), antimicrobial susceptibility testing, and whole-genome sequencing were employed to identify and analyze the isolates.Results and discussionAll six isolates were carbapenemase-producing K. pneumoniae isolates with chromosomally carried blaSHV, fosA, oqxA and oqxB genes, as well as nine to 21 virulence genes. The isolates belonged to five multilocus sequence types (STs): one isolate from a human and one from a dog belonged to ST16, with the other two human isolates being from ST340 and ST1269 and the other two dog isolates were ST147 and ST15. One human isolate and two dog isolates harbored the same blaOXA-232 gene on the ColKP3 plasmid, and one dog isolate carried the blaOXA-48 gene on the IncFII plasmid. Notably, one human isolate exhibited resistance to colistin mediated by the mcr-3.5 gene carried on the IncFII plasmid, which co-existed with resistance determinants to other antibiotics, including aminoglycosides and quinolones. In conclusion, this study provides a comprehensive characterization of both chromosome- and plasmid-mediated carbapenem and colistin resistance in a set of K. pneumoniae clinical isolates from unrelated humans and dogs in Thailand. The similarities and differences found contribute to our understanding of the potential widescale dissemination of these important resistance genes among clinical isolates from humans and animals, which in turn may contribute to outbreaks of emerging resistant clones in hospital settings.

OXA-232 is one of the most common OXA-48-like carbapenemase derivatives and is widely disseminated in nosocomial settings across countries. The bla OXA-232 gene is located on a 6-kb non-conjugative ColKP3-type plasmid, while the dissemination of bla OXA-232 into different Enterobacterales species and the polyclonal dissemination of OXA-232-producing K. pneumoniae revealed the horizontal transfer of bla OXA-232 . However, it’s still unclear how this non-conjugative ColKP3 plasmid could facilitate the mobilization of bla OXA-232 . Here, we observed the in vivo intraspecies transfer of bla OXA-232 during a nosocomial outbreak of OXA-232-producing K. pneumoniae . We demonstrated the presence of ColKP3 OXA-232 plasmid in the outer membrane vesicles (OMVs) derived from clinical isolates, and OMVs could facilitate the horizontal transfer of bla OXA-232 among Enterobacterales . In contrast, for the most prevalent carbapenemase genes, including bla KPC-2 and bla NDM-1 , though the presence of carbapenemase genes and plasmid backbones in the vesicular lumen was observed, OMVs couldn’t promote effective transformation, probably due to the low copy number of plasmids in clinical isolates and the low number of plasmids loaded into vesicles. Conjugation assay revealed that the epidemic IncX3 NDM-1 and IncFII(pHN7A8)/IncR KPC-2 plasmids were conjugative and could be horizontally transferred via independent conjugation or with the help of a co-existent conjugative plasmid. For the large-size and low-copy number conjugative plasmids carrying carbapenemase genes, OMVs-mediated gene exchange may only serve as an alternative pathway for horizontal transfer. In conclusion, diverse mobilization strategies were employed by plasmids harbouring carbapenemase genes, and plasmids display a proper choice of mobility pathway due to their individual properties.

Klebsiella pneumoniae is a type of gram-negative bacterium that was initially discovered and isolated by Carl Friedlander in the year 1882. Klebsiella pneumoniae is responsible for a range of nosocomial and community-acquired illnesses, including urinary tract infections (UTIs), pneumonia, surgical site infections, and bloodstream infections. According to a number of studies, gram-negative sepsis ranks as the second most prevalent cause. A comprehensive analysis of the entire genome of Klebsiella pneumoniae was conducted using Illumina sequencing by synthesis (SBS) technology, specifically the Illumina HiSeq platform. The genetic material of the organism was comprised of a single circular chromosome of three thousand base pairs in length, exhibiting a GC content of 55.90%. In total, the genome exhibited a collective count of 164 genes associated with transfer RNA, 9 genes associated with ribosomal RNA, and 11,132 protein-coding sequences. Moreover, the analysis of genetic diversity entailed the application of sequence data, which was then compared to the reference genome that shared the highest degree of similarity. The genome annotation service offered by PATRIC and the RAST annotation system have successfully detected many antimicrobial resistance elements, clusters of genes associated with antibiotic resistance, as well as efflux pumps. This study represents the first known case of conducting a comprehensive genome sequencing of Klebsiella pneumoniae within the geographic area of Iraq, based on our current knowledge. The current investigation holds potential for advancing our comprehension of the antibiotic resistance traits demonstrated by Klebsiella pneumoniae, therefore enabling the healthcare facility to efficiently handle and alleviate outbreaks.
