Introduction: Patella fractures account for 0.5% to 1.5% incidence of all bone fractures. The gold standard procedure for the patella fractures so far is AO modified anterior tension band wiring with stainless steel and K-wires. The hardware related complication in this technique is high ranging from 0-60%. The purpose of the study is to evaluate the functional outcome for fixation done with high resistance suture material. Methods: A prospective study was conducted in a group of 24 patients who were hospitalised in Govt Kilpauk Medical College and Hospital, Chennai in the Department of Orthopaedics between August 2015 and September 2017. The patella fractures included in the study were performed open reduction and internal fixation (ORIF) with FiberTape using a novel transosseous tunnel technique. The functional outcomes were evaluated with Tegner-Lysholm score and Bostman scoring. The clinical and radiological outcomes were evaluated as well. Results: Among the 24 patients with patella fractures studied, all fractures united. The mean time of union is 3 months 8 days. 4 patients presented with minor complications like superficial infection, displacement < 4mm, anterior knee pain and extensor lag ~ 10°. Average scores: Lysholm -96, Bostman -27. All fractures united uneventfully. No hardware related complications or resurgery reported in the study. Conclusion:The complete non-metallic fixation rules out the most important complication of metallic fixation, which is the symptomatic hardware. It also brought down the reoperation rates and the infection rates post-surgery considerably. Using a non-absorbable suture material also precludes the possibility of a second surgery needed for the implant exit. Hence, these high resistance suture materials like Fiber Wire and FiberTape can be used for efficient non-metallic fixation of patella on par with that of traditional metallic fixation with reduced complications and re-surgery rates.

Relatou-se a ocorrência de fratura patelar associada à ruptura do tendão patelar em um cão sem raça definida, macho, atendido em um hospital veterinário escola. A técnica utilizada foi a combinação de fixação óssea com fio de Kirschner e aplicação de banda de tensão, recomendada em fraturas transversais da patela. Para reparar a lesão do tendão patelar, realizou-se a sutura tipo Kessler modificada com fio de náilon 0,80mm. No retorno do paciente, 150 dias após o procedimento cirúrgico, havia processo cicatricial ósseo avançado da patela. Concluiu-se que as medidas adotadas para reparação da lesão foram eficazes

Comparison of the outcomes of pull through suturing vs. tension band wiring fixation techniques in the management of patellar fractures

Background: Operative fixation of displaced patella fractures has now become the standard of care. This study aims to asses functional outcomes, as well as identify complications in a patients treated with non-absorbable braided suture fixation for patellar fractures. These patients were then compared to a group of patients treated with K-wires with tension band wiring.Methods: This study was conducted on the patients admitted to orthopaedic ward at MGM medical college and hospital, Kamothe, Navi Mumbai. Total number of patients was taken for this study. Patients were followed up for a period of 6 weeks, 3 months, 6 months and at each visit. Patients were assessed by Lysholm knee score.Results: The mean age was 43.33 years for TB wiring group and 46.26 years for pull through suturing group. Out of 30, 15 were in TB wiring group in which 14 males and 1 female and, 15 in pull through suturing group 4 females and 11 males. The mean Lysholm knee score at 6 weeks for TB wiring group was 68.2 at 3 months was 81.4 at 6 months was 87. The mean Lysholm knee score for 6 weeks for pull through suturing was 69.2, at 3 months was 72.5 and at 6 months was 79.2.Conclusions: Patella fracture most common fractures, in this study we have discussed two modalities of treatment one is TB wiring and second is pull through suturing showing results that TB wiring have better results.
