A new aortoventriculoplasty for prosthetic aortic valve replacement

Abstract: The described aortoventriculoplasty has several advantages, including: a simple exposure obtained by partly separating the pulmonary artery valve from the right ventricle; clear septal opening landmarks that avoid the conducting tissue; easy reconstruction with a single septal patch; and an anatomically restored right ventricular outflow tract.

“…Results with aortic homograft replacement in this age population are fraught with high calcification rates and are discouraging. Alternatively, reconstruction with a mechanical valve and modified aortoventriculoplasty9 is possible but would preclude future pregnancy. While macrolides have traditionally been indicated in the treatment of Bartonella infections, it seems that employing aminoglycosides for 2 weeks optimises therapy and is associated with better outcomes 2…”

“…Aiming to decrease the incidence of heart block and complexity associated with Konno’s technique, Hvass and colleagues 12 applied the technique of pulmonary autograft harvest to aortoventriculoplasty. In this technique, the septal opening is in a different location compared to the Konno operation, and is away from the atrioventricular conducting tissue.…”
