A New Method for the Treatment of Macrocephaly Caused by Hydrocephalus

Abstract: The combination of these modalities represents a unique state-of-the-art method for the correction of severe macrocephaly without the risks of intracranial hemorrhage and provides a useful adjunct to the treatment of hydrocephalus.

“…rocephaly due to hydrocephalus and that improvement of patient functioning and aesthetics could be achieved after surgery. 6,7,10,13,17 We reasoned that a simulated computer program to reconstruct the head prior to surgery should be used and, together with the aforementioned surgical techniques, could provide good aesthetic outcomes postoperatively. We also used a sagittal bandeau as described by Sundine et al 13 in all of our total cranial vault remodeling procedures for the correction of hydrocephalic macrocephaly (Figs.…”

“…To date, the technical details and outcomes of using surgical methods for the treatment of hydrocephalic macrocephaly remain the focus of intensive discussions. 3,4,6,7,11,15,16,18 In this study, the clinical profile and outcome of the children with hydrocephalic macrocephaly who had undergone shunt surgeries with 1-stage total cranial vault remodeling were analyzed retrospectively. In addition, the feasibility of surgery for total cranial vault remodeling in children with hydrocephalic macrocephaly is discussed.…”

“…Many authors report placement of VP shunts or subcutaneous ventri cle-tapping reservoirs prior to definitive macrocephaly treatment. 4,6,7 We advocate the use of a CSF shunt with a programmable valve in this setting. This allows for con tinued management of the hydrocephalus and manage ment of postoperative subdural hematoma formation in this highrisk group.…”

“…[2][3][4][7][8][9] Previous reports describe large holocalvarial reconstructions with substantial risk of hemorrhage, transfusion, prolonged time on the operating table, and multiple stages. Our technique limits the en tire process of reduction cranioplasty/fixation to 2 steps: placement of a CSF shunt, followed by a single cranial remodeling and fixation procedure that takes less than 1 hour.…”

“…Newer reported techniques include limited reduction cranioplasty focused on correcting posterior turricephaly, 7 single-stage total cranial vault remodeling with the addition of prolonged helmet therapy, 8 and use of resorbable plating systems to aid in reconstruction. 9,10 Distraction osteogenesis (DO) was first utilized in craniofacial surgery as a technique for mandibular lengthening and later expanded upon as a technique for midface advancement and cranial vault remodeling. 11 In the cranial vault, DO is often used for posterior vault remodeling in the treatment of craniosynostosis, as the gradual expansion minimizes dead space and allows for concomitant expansion of the dura and soft tissues.…”
