A new technique of combined retinal and choroidal biopsy

Abstract: The technique of large chorioretinal biopsy described appears to be safe. It produced good amounts of chorioretinal tissue for histopathological analysis. Positive histology results were seen in the majority of the sample and especially in those where vitreous biopsy alone proved to be inadequate.

“…The risk of complications, such as vitreous hemorrhage and retinal detachment, mean a strong clinical indication for biopsy is required. 7,9 In our case, we believe it was appropriate to proceed to chorioretinal biopsy to establish a firm microbiological diagnosis in a patient demonstrating minimal improvement after conventional treatment.…”

“…7,8 A number of different techniques have been developed, including fine needle aspiration, endoretinal biopsy, and transscleral external biopsy. 7 Although modern techniques are considered relatively safe with good diagnostic results, they are not without complications. “…The presence of a subretinal or chorioretinal lesions can require biopsy in unknown cases where disease progression occurs despite therapy (Martin et al 1993 ;Cole et al 2008 ;Lim et al 2005 ;Johnston et al 2004 ). There are multiple approaches to obtaining a choroidal/subretinal biopsy including transvitreal, fi ne-needle aspiration, and external choroidal biopsy.…”

“…At this point, multiple different techniques can be employed (Cole et al 2008 ;Johnston et al 2004 ;Sen et al 2006 ; Gonzales and Chan 2007 ). The area for biopsy can be treated with endodiathermy to reduce the risk of bleeding.…”

“…Various surgical methods have been applied since Hirschberg first performed an intraocular biopsy in 1868 [3]. These surgical procedures will not be the subject of this chapter, and the readers are referred to relevant reviews [4][5][6][7][8][9][10][11]. Essentially, the bioptic targets include all intraocular structures, such as the iris, ciliary body, choroid, retina as well as the aqueous and vitreous humour.…”
