A non-invasive instrument for clinical assessment of the pre-corneal tear film stability

Abstract: A simple, non-invasive technique has been developed for assessment of the stability of the pre-corneal tear film. Changes are observed in the reflection of a grid pattern from the tear film surface. Breaks in the tear film appear as random discontinuities in the grid image. Using this non-invasive technique the stability of the pre-corneal tear film was assessed in nine normal subjects and twelve established dry-eye patients. The non-invasive tear film break-up time (NIBUT) of the dry-eye patients was on avera… Show more

“…64 According to this classification, NIBUT, nasal LIPCOF, and LIPCOF Sum were excellent discriminators, and TMH, PRTT, temporal LIPCOF, and LWE were acceptable discriminators. These results are not surprising, since NIBUT, 13,65 TMH, 58,66 PRTT, 46,47 LIPCOF, 13,16,18,19 and LWE 15,18,19 are accepted tests in dry eye assessment. Even though bulbar hyperaemia approached significance (P ¼ 0.055), its AUC was too small to be a predictive discriminator.…”

“…A reference value of Ͻ10 seconds for TBUT is used in the diagnosis of dry eyes. 11 The TBUT has been shown to be dependent on the reduction of surface tension by mucins and other surface-active agents but not so much on the quantity of aqueous content in tears. 12,13 Using the above value 97% of the patients had an abnormal TBUT ( Figure 5).…”

“…Moreover, we measured the tear film break up time by a noninvasive method (NIBUT) by ocular microscopy using a small eye microscope (Keeler TearScope; Keeler Instruments, Windsor, UK). The instrument projects a grid of equidistant circles of light on the eye surface that are blurred by tear film break up (Mengher et al, 1985). The time until the blurring of the lines can be observed is counted.…”
