A Novel Approach Incorporating Sildenafil in the Management of Symptomatic Neonates with Ebstein’s Anomaly

Abstract: The neonatal presentation of Ebstein's anomaly is a distinct cyanotic lesion for which little can be done other than to maintain ductal patency or perform palliative surgery if improvement does not occur. We report two cases in which sildenafil was introduced to assist previously failed attempts at weaning of prostaglandin.

“…11 Our present report suggests a potential new indication for sildenafil-that of improving pulmonary blood flow in neonatal EA. Similar results were reported by Pham et al 12 except that they used a higher dose (1 mg/kg/dose) and for a longer duration (32 weeks). In our case, a 6-week course of sildenafil was successful, although the duration of therapy may depend on the degree of initial pulmonary vascular resistance and the time it takes to drop to normal levels.…”

“…34,35 Sildenafil has been used in a similar manner to decrease PVR and promote antegrade pulmonary blood flow. 36 Ventilation strategies to optimize PVR by minimizing hypercarbia and acidosis are beneficial. For patients with cyanosis and either severe pulmonary valve stenosis or pulmonary valve atresia, maintaining ductal patency is necessary.…”
