BackgroundThe aim of the current study was to explore the anti-arthritic effect of pinitol via assessing its effect on various inflammatory mediators and its possible mechanism of action.Material/MethodWe assessed the anti-arthritic effect of pinitol in a formaldehyde- and CFA-induced arthritic model in Wistar Swiss albino strain rats divided into 6 groups. The rats received different doses of pinitol and indomethacin for 28 days. The arthritic index and body weight were determined at regular intervals, together with hepatic, hematological, and antioxidant parameters. The expression of proinflammatory cytokines (e.g., IL-6, TNF-α, and IL-1β) and inflammatory mediators (e.g., COX-2 and VEGF) were also estimated with histopathological evaluation of the joint tissue of rats. A docking study of pinitol with PTPN22 was also carried out.ResultsThe CFA-induced model rats developed redness and nodules in the tail and front paws, and the arthritic control (AC) group rats showed similar symptoms, which were decreased by pinitol administration. The body weight of AC group rats was decreased, while pinitol-treated rats showed considerably increased body weight. Hematological, hepatic, and antioxidant parameters were altered by pinitol in a dose-dependent manner. Pinitol significantly decreased the elevated concentration of proinflammatory cytokines and inflammatory mediators, with improvement in histopathological condition. The docking study suggested that pinitol efficiently interacted with PTPN22 via Arg59, Tyr60, Leu106, and Lys138 by creating close interatomic hydrogen bonds and hydrophobic contacts.ConclusionsPinitol showed anti-arthritic effects via reduction of proinflammatory cytokines and inflammatory mediators via inhibition of PTPN22.

A B S T R A C TRheumatoid arthritis (RA) is the most common complex multifactorial joint related autoimmune inflammatory disease with unknown etiology accomplished with increased cardiovascular risks. RA is characterized by the clinical findings of synovial inflammation, autoantibody production, and cartilage/bone destruction, cardiovascular, pulmonary and skeletal disorders. Pro-inflammatory cytokines such as IL-1, IL-6, IL-8, and IL-10 were responsible for the induction of inflammation in RA patients. Drawbacks such as poor efficacy, higher doses, frequent administration, low responsiveness, and higher cost and serious side effects were associated with the conventional dosage forms for RA treatment. Nanomedicines were recently gaining more interest towards the treatment of RA, and researchers were also focusing towards the development of various anti-inflammatory drug loaded nanoformulations with an aid to both actively/passively targeting the inflamed site to afford an effective treatment regimen for RA. Alterations in the surface area and nanoscale size of the nanoformulations elicit beneficial physical and chemical properties for better pharmacological activities. These drug loaded nanoformulations may enhances the solubility of poorly water soluble drugs, improves the bioavailability, affords targetability and may improve the therapeutic activity. In this regimen, the present review focus towards the novel nanoparticulate formulations (nanoparticles, nanoemulsions, solid lipid nanoparticles, nanomicelles, and nanocapsules) utilized for the treatment of RA. The recent advancements such as siRNA, peptide and targeted based nanoparticulate systems for RA treatment were also discussed. Special emphasis was provided regarding the pathophysiology, prevalence and symptoms towards the development of RA.

SUMMARY The human gastrointestinal tract is resident to a vastly diverse microbial consortium that co-exists through strict rules of invasion, dominance, resilience and succession. While some members possess stronger capabilities for survival than others, each one retains a genome characteristic of their bacterial denomination which subsequently determines survival and ultimately the composition of a human gut microbiome. Collective evidence advocates the concept of gut microbiota modulation via dietary compounds, with or without nutraceutical supplementation. However, consistent reports of strong individuality in responsiveness suggest that initial composition of host microbiota mediates the effect of nutrition modulation. There is also a strong potential for the interaction between mind and microbe to influence responsiveness, although mechanistic understanding of these complex exchanges remains in its infancy at best. Synthetic stool for FMT is a next-generation microbiome-therapy shown effective in treating C.difficile [417] which could provide a feasible alternative to current methods for patients with IBD. Nevertheless, studies investigating optimum timing for FMT administration are essential. Animal and human studies are only starting to highlight the Pandora of interactions that endure between members of gut microbiome, their associated metabolites, dietary compounds, as well as host neurological and immune systems, all of which characteristic to each individual. Advanced research technologies have excelled the scientific evidence in support of CAM and toward generating NG-CAM systems designed for treatment of specific disease states, such as IBD. While the majority of envisioned NG-CAM strategies presently exist in their experimental and discovery phases, many show promise for future clinical application.
