A Novel Paradigm for Heart Failure With Preserved Ejection Fraction

Abstract: Over the past decade, myocardial structure, cardiomyocyte function, and intramyocardial signaling were shown to be specifically altered in heart failure with preserved ejection fraction (HFPEF). A new paradigm for HFPEF development is therefore proposed, which identifies a systemic proinflammatory state induced by comorbidities as the cause of myocardial structural and functional alterations. The new paradigm presumes the following sequence of events in HFPEF: 1) a high prevalence of comorbidities such as over… Show more

“…A novel paradigm in HFpEF associates comorbid conditions to an inflammatory state that increases LV stiffness and promotes HF 71. The cellular and molecular mechanisms that lead from systemic inflammation to myocardial fibrosis (reduced nitric oxide bioactivity, cyclic guanosine monophosphate content, and protein kinase G) also promote arterial stiffening, with the endothelium being an essential component of vascular homeostasis 72, 73, 74, 75.…”

“…Since fibrosis and inflammation are believed to be part of the pathogenesis of HF with preserved ejection fraction (HFpEF),5 MRA is an appealing treatment also for this patient group. The TOPCAT (Treatment of Preserved Cardiac Function Heart Failure With an Aldosterone Antagonist) study of spironolactone in HFpEF did not show any positive effects on mortality, although a post hoc regional analysis indicated beneficial effects in the American patients 6, 7.…”

“…The clinical implications of our current findings can be gauged against the prognosis associated with asymptomatic diastolic LV dysfunction, which in the general population has a prevalence of around 25%4, 5 and over time evolves to symptomatic LV dysfunction6 and heart failure2 and predicts the incidence of adverse health outcomes. Guideline‐driven echocardiographic criteria24, 25 to stage patients with advanced diastolic LV dysfunction leave a large proportion of people unclassified in population studies.…”

“…Diastolic HF, also referred to as HF with preserved ejection fraction, accounts for 50% of cases 2. Mortality of diastolic HF is 30% within 1 year of the first hospital admission 3.…”
