A Novel Variant, NDM-5, of the New Delhi Metallo-β-Lactamase in a Multidrug-Resistant Escherichia coli ST648 Isolate Recovered from a Patient in the United Kingdom

Abstract: A new variant of the New Delhi metallo-enzyme (NDM) carbapenemase was identified in a multidrugresistant Escherichia coli ST648 isolate recovered from the perineum and throat of a patient in the United Kingdom with a recent history of hospitalization in India. NDM-5 differed from existing enzymes due to substitutions at positions 88 (Val3Leu) and 154 (Met3Leu) and reduced the susceptibility of E. coli TOP10 transformants to expanded-spectrum cephalosporins and carbapenems when expressed under its native promot… Show more

“…Mussi et al (2005) have proved that ISAba125 causes insertional inactivation of the CarO OMP in A. baumannii strains, leading to a carbapenem-resistant profile. The insertion element ISAba125, first identified in A. baumannii, is widely distributed and has also been associated with gene variants of the bla NDM carbapenemase in the family Enterobacteriaceae (Hornsey et al, 2011;Nordmann et al, 2012;Pfeifer et al, 2011). The results obtained in this study show that IS elements such as ISAba125 could modulate antibiotic resistance depending on the antibiotic treatment of the patient as the isolates used in this study were of clinical origin.…”

“…The E. coli strains that were isolated from the media containing carbapenem were further analyzed to determine the mechanisms involved in resistance to carbapenems using a multiplex PCR (Dallenne, Costa, Decré, Favier, & Arlet, 2010; Hornsey, Phee, & Wareham, 2011). Briefly, the presence of the most prevalent carbapenemase genes (including bla KPC , bla VIM , bla OXA‐48 group, and bla IMP ) was assessed by multiplex PCR, as previously described (Dallenne et al., 2010) and bla NDM gene presence was investigated by PCR assay (Hornsey et al., 2011).…”

“…ST405 with various types of CTX-Ms has a worldwide distribution (7,14,19,35), while ST648 with CTX-M-15 and -32 is present in poultry from Spain (8) and humans in China (44) and the Netherlands (41). ST405 and ST648 are also associated with NDM ␤-lactamases (11,21).…”
