A paradigm shift in eye banking: how new models are challenging the status quo

Abstract: This article presents a fact-based, direct approach in order to evaluate the strengths and short comings of both local eye banks and larger companies that have penetrated the market of eye donation. By taking a non-biased approach to the topic, the goal of this article is to create a platform, which furthers the discussion. We focus on SightLife and CorneaGen as well as local eye banks and how their models differ in terms of tissue procurement, distribution, and surgeon partnership.

“…For instance, Lions Eye Bank consists of more than 60 eye banks worldwide, all operated by a non-profit foundation. 26 In contrast, CorneaGen serves as a for-profit business entity with the goal of eliminating corneal blindness. 26 The Eye Bank Association of America (EBAA) represents 83 eye banks across the United States as well as 14 international eye banks.…”

“…Since then, very large organizations have emerged in the United States, delivering > 35,000 transplants per year. However, there is controversy as to whether corporate structures of this size can genuinely still improve outcomes or whether they are creating new problems in an ethically sensitive field [52,53]. It has been suggested, for example, that complicated supply chains could affect emergency supplies due to local bottlenecks.…”

“…At the same time, despite larger and more centralized structures, there has been no measurable increase in nationwide acquisition. An additional argument is that near-industrial utilisation turns tissue donation into a commodity [52,54]. In light of this, Germany is currently experiencing a trend towards an even greater degree of collaboration between independent cornea banks in the future.…”

“…I read, with interest, the article by Moshirfar et al1 examining the shift in eye banking models that is occurring in our country. While the authors maintain that their purpose is to provide a fact-based, non-biased exposition of the current issues, they fall regrettably short of this goal.…”
