A Peculiar Cause of Anaphylaxis: No More steak?

Abstract: In recent years, a newly recognized allergic disease has been uncovered, and seemingly idiopathic causes of anaphylaxis now have an explanation. Individuals bitten by the lone star tick may develop IgE antibodies to the carbohydrate galactose-α-1,3-galactose (alphagal). Upon exposure of sensitized subjects to mammalian meat containing alpha-gal on glycoproteins or glycolipids, delayed anaphylaxis may ensue, often three to six hours after ingestion. 1 Many of these individuals have negative allergy skin prick t… Show more

“…The loss of alpha gal may have made Old World primates resistant to infection, because anti alpha gal antibodies target this foreign epitope on enveloped viruses coming from other mammalian species [64]. Anti alpha gal antibodies can cause severe allergic reactions in humans that eat red meat after immunization by a tick bite [65]. Alpha gal antibodies also prevent organ transplantation from other mammal species – pig organs are rejected partly because they are rich in alpha gal [66].…”

“…The alpha-gal allergy is a newly discovered condition that provides diagnostic challenges (23,24). This novel, severe food allergy to mammalian meat has a highly unusual presentation; unlike most food allergies, there is a delayed reaction between ingestion and symptom onset, usually 3 to 6 hours postconsumption (25).…”

“…10,14 Often there is a positive history of a prior tick bite. Bites from A americanum can be severely pruritic, 19 a symptom that can last weeks, 10 making them memorable. Bites might not be recognized as "tick" bites by some patients.…”
