A pilot study of therapeutic plasma exchange for serious SARS CoV-2 disease (COVID-19): A structured summary of a randomized controlled trial study protocol

Abstract: Objectives To evaluate the safety of therapeutic plasma exchange (TPE) in adult patients with serious/life-threatening COVID-19 requiring intensive care unit (ICU) admission, and associated 28-day mortality. Serious and life threatening COVID-19 are defined as per published literature (please, refer to the full protocol, Additional file 1). The rationale is that TPE can remove interleukins-3, 6, 8, 10, interferon-gamma and tumor necrosis factor-alpha. Thus, it may reduce the cytokine release syndrome associate… Show more

“…The inclusion criteria for the application of TPE as rescue therapy in life-threatening COVID-19 are detailed elsewhere. 16 Briefly, adult (<18 years old) mechanically ventilated patients with confirmed SARS-CoV-2 infection and life-threatening features such as ARDS (according to the Berlin criteria), Acute Physiology and Chronic Health Evaluation II score ⩾20, severe sepsis/septic shock, MSOF, and associated CRS were eligible for TPE. 16 CRS was defined based on the criteria outlined in Table 1.…”

SARS-CoV-2 infection was confirmed by reverse transcriptase polymerase chain reaction (RT-PCR) assays performed on nasopharyngeal swabs using QuantiNova Probe RT-PCR kit (Qiagen) in a Light-Cycler 480 real-time PCR system (Roche, Basel, Switzerland).…”

“…Применение плазмообмена обосновано возможностью удаления цитокинов, связанных с патогенезом цитокинового шторма при COVID-19 [6]. Применение иммуноглобулина повышает неспецифическую резистентность организма после проведения иммуносупрессивной терапии, показано при всех тяжелых вирусных и бактериальных инфекциях.…”

“…Our group's interest about the putative benefit of therapeutic plasma exchange (TPE) in critically ill patients with SARS-CoV-2 disease (COVID-19) has been documented. 2 In a minority of patients, COVID-19 can present with life-threatening features such as acute respiratory distress syndrome (ARDS), septic shock, thromboembolic disease, multi-system organ failure, and associated cytokine release syndrome (CRS). The latter is defined by criteria outlined in Table 1 .…”
