A presumed extragonadal germ cell tumor that turned out to be a gastric cancer—a case report

Abstract: A solely retroperitoneal mass in males in combination with elevated serum Alpha-Fetoprotein (AFP) and beta-human choriogonadotropin (β-HCG) levels is highly indicative of a metastatic testicular cancer. Although testicular cancers are rare, they represent the most common diagnosed cancer in males between 14 and 40 years. However, in cases without evidence of a primary testicular tumor, the rare diagnosis of a retroperitoneal extragonadal germ cell tumor (EGCT) must be assumed. Here, we describe the first publi… Show more

“…AFP-producing malignancies of the ovaries or gastrointestinal tract were with a poor prognosis. 22,23 In this case, FGFR3, GNAS, and ZNF217 mutations might be the reason for the AFP-producing process and provide a hint for the potential therapeutic target. Chen et al 24 reported that EBER-positive signals were detected in 91.43% patients with non-keratinizing NPC in China.…”

“…Hence, immunohistochemical examination was carried out for pancytokeratins, markers of tumor cells of epithelial origin, and caudal related homeodomain transcription 2, which is ectopically expressed in intestinal metaplasia and is a marker of intestinal-type gastric adenocarcinomas[ 26 ]. Several studies have reported that immunohistochemically-confirmed gastric cancer patients have elevated serum levels of β-HCG[ 27 , 28 ]. Although the mechanisms of β-HCG production in gastric cancer cells are unknown, it is known that serum β-HCG can be positive in some patients with gastric cancer.…”
