“…The higher incidence of suicidal attempts in patients with acute phosphide poisoning than other modes of poisoning could be attributed to the increasing stresses today facing people particularly the youth including the stresses of unemployment, economic instability and failure of education, love or work. Objective: To evaluate potential oxidative stress in patients with acute phosphide poisoning and the effect of vitamin C. Methods:Participants were females and divided into three groups; group I: healthy volunteers group II: healthy volunteers received vitamin C, group III: patients with acute phosphide poisoning received the supportive and symptomatic treatment and group IV: patients with acute phosphide poisoning received the supportive and symptomatic treatment in addition to vitamin C. All the participants were subjected to thorough history, clinical examination, ECG and laboratory investigations were carried on collected blood and gastric lavage samples on admission. Blood samples were divided into two parts, one for measurement of routine investigations and the second part was used for evaluation of malondialdehyde and total thiol levels before and after receiving the treatment regimen.Results: Most of the cases in this study were among the age group of 15-25 years, females, single, secondary school education, from rural areas and suicidal. All vital signs were within normal range and the most common complaint was vomiting and abdominal pain. All cases in this study showed normal routine investigations. The mean MDA levels after receiving treatment decreased significantly in groups II and IV. The mean total thiol levels increased significantly after receiving treatment in groups II and IV. Conclusion:It can be concluded that vitamin C has a potential benefit due to its antioxidant property on zinc phosphide induced-oxidative stress in acute zinc phosphide poisoned patients.

StreszczenieCel pracy: Ustalenie profilu demograficznego, cech klinicznych oraz analiza trucizny w warunkach klinicznych. Badanie przeprowadzono w Sri Aurobindo Medical College oraz w PG Institute w Indore (stan Madhya Pradesh). Łącznie przeanalizowano 75 przypadków zatruć pod kątem danych demograficznych, parametrów życiowych (ciśnienie krwi, puls, tętno, źrenice itd.), cech klinicznych (np. wymioty, ślinienie, przytomność itd.), rodzaju trucizny oraz jej analizy. Wyniki: Zatrucia występowały częściej w grupie osób w wieku 15-25 lat, częściej u mężczyzn niż u kobiet oraz u wyznawców hinduizmu. Dochodziło do nich głównie na obszarach wiejskich oraz u osób żyjących w związkach mał-żeńskich. Większość pacjentów zwolniono do domu po przeprowadzeniu odpowiednich procedur terapeutycznych i udzieleniu porady, u większości stwierdzono zmienione parametry życiowe i cechy kliniczne. Ocenie poddano związ-ki fosforoorganiczne oraz fosforek glinu. Wnioski: Działania profilaktyczne należy realizować poprzez edukację, odpowiednie doradztwo, wspieranie ośrodków informacji o zatruciach i tworzenie oddziałów toksykologicznych w szpitalach.Słowa kluczowe: zatrucie, profil demograficzny, cechy kliniczne, analityczne. AbstractAim of the study is to find out demographic profile, clinical characteristics and analysis of poison in clinical set up. The study carried out in Sri Aurobindo Medical College and PG Institute Indore, Madhya Pradesh. Total 75 cases of poisoning were studied for demographic profile, vitals (BP, pulse, heart rate, pupils, etc.), clinical features (such as vomiting, salivation, consciousness, etc.), type of poison and its analysis. Results: Poisoning was more common in cases between 15 and 25 years of age, in males than in females and in Hindu religion. Poisoning cases were predominantly from rural areas and in married people. Majority of cases were discharged after proper treatment and counseling. Altered vitals and clinical features were found in most of the cases. Organophosphate and aluminum phosphide compound were evaluated in most of the cases. Conclusions: Preventive measures should be applied through educating people, proper counseling, promoting poison information centers, and introducing separate toxicological units in hospitals. Background: Drug overdose can be accidental or deliberate posing a significant source of morbidity, mortality and health care burden. Drug over doses may be seen at any age and is particularly common among teenagers. It is estimated that approximately 2-3 million drug poisonings occur every year in the United States. In India the true incidence of self drugpoisoning is unknown due to under diagnosis and under reporting. Acute self drug poisoning is mostly due to benzodiazepines, antidepressants, antiepileptics and polypills containing NSAIDs. There is scarcity of data from the Indian subcontinent regarding the profile and outcome of patients presenting with acute self drug poisoning. We undertook this retrospective analysis to assess the course and outcome of such patients admitted in our tertiary care teaching hospital. Methods: This retrospective study was conducted between Jan 2008-Dec 2011 in the Department of medicine, Sri Devaraj Urs Medical College, Kolar. The data retrieved from the medical records section was analyzed for 155 patients who were admitted with acute self poisoning for, type of poisoning, time of presentation, reason for the consumption of drugs, ICU admission if required and outcomes. Results: The majority were females 103 (66.4%) patients and 52 were males and majority 90 (58 %) of admissions were from age group 21 to 30 years. The most common drugs consumed was benzodiazepines 54(34.8%), followed by antiepileptics 41(26.4%), and poypills 33(21.3). Thirty (19.4%) consumed two or more drugs. Commonest cause for drug consumption was suicidal (90%), and was mainly oral. The highest incidence of toxicity was due to benzodiazipines (34.8%) followed by antiepileptics (26.4%). Ten patients (15.5%) required ICU management for ventilatory support and average stay was seven days. None of our patients in the study required ionotropes or dialysis. Conclusions: The present data is an insight of the trends in self drug poisoning in rural south India as a result of low socioeconomic status, illiteracy and easy availability of drugs over the counter. Therefore strict regulations enforced on the medical drug outlets with mental health awareness counselling/programmes can go a long way in curtailing this problem.
