Objective: Ruptured saccular aneurysms are a common and serious medical problem. The prevalence of aneurysms is low during the first two decades of life and increases steadily after the third decade. Anterior communicating artery (AComA) aneurysms are regarded as the most complex of all intracranial aneurysms.Methods: Here, we report a case of an aneurysm of AComA patient with the development of microsurgical technique and intraoperative monitoring technique.Results: Permanent clipping was successfully performed in the neck of an aneurysm of AComA.Conclusion: Microsurgical clipping still remains a definitive treatment of ruptured cerebral aneurysms.

Objective: The objective of the study was to find out differences in the cortisol level characteristics and National Institutes of Health Stroke Scale (NIHSS) score in the 1st and 7th days on patients treated at Dr. Soetomo Regional Hospital; thus, it can be the right education for stroke patients.Methods: A prospective observational study was carried out against stroke patients treated in the neurosurgery department on September 2016– March 2017. Evaluation was done to all patients treated at the neurosurgery department. The evaluation process began with basal hormones within the first 24 h and taken at 6:00 and repeated on the 7th day to surviving patients. Of all the data obtained, the category scale data are presented by its frequency distribution, while numerical scale data are presented by its descriptive value (average and standard deviation).Results: Of the 17 patients surviving until the 7th day, there is a decrease at the average cortisol level on the 1st day when compared to the average cortisol level on the 7th day with a mean value of 39.67–36.57 and so is the highest cortisol level; it decreases from the 1st day of 61.45 to the 7th day of 56.70. Meanwhile, in the NIHSS score value, the decrease in score only occurs in the average value on the 1st and 7th days from an average of 22.82 into 21.41, whereas minimum and maximum scores do not change. With the test result, the cortisol level characteristic gets a score of p<0.05; thus, there is a significant difference.Conclusion: Of the result of observation already carried out on stroke patients treated at Dr. Soetomo Regional Hospital, treatment and education levels for patients who have a risk of stroke factor are very important.

Is There Any Benefit of Citicoline for Acute Ischemic Stroke ? Systematic Review of Current Evidences

Two main strategies have been suggested for the treatment of ischemic stroke including reperfusion and neuroprotection. Citicoline is one of neuroprotector agent that commonly used in ischemic stroke. A systematic review of citicoline focusing only on ischemic stroke has not yet performed. This systematic review aimed to identify the effectiveness of citicoline in patients with ischemic stroke history. A systematic research was performed using PubMed and Cochrane Library as the database. Keywords were: "citicoline and stroke", "citicoline and ischemic stroke", and "randomized controlled trial citicoline". The inclusion criteria were: (i) the trials conducted between 2007 to 2017, (ii) written in English, (iii) the studies performed in human, (iv) patients had ischemic stroke history, (v) citicoline was the main drug assessed in the study. The exclusion criteria i.e.: (i) the study was not a randomized controlled trial (RCT) and (ii) the full text was not available. The quality of RCT was determined by using Jadad score. A literature search revealed 541 journals from PubMed and Cochrane Library. After screening the title and abstract, adjusting for inclusion and exclusion criteria, adjusting for the irrelevant topic, and duplication the final result was 4 RCT studies. All studies had a good quality, indicated by Jadad score of 3 or more. Three studies conclude that no statistically significant difference in treatment outcome between citicoline and other groups. The last study revealed citicoline is effective in preventing post-stroke cognitive impairment.
