Introduction: Thyroid swellings are commonly encountered endocrine disorders. These disorders can be evaluated by thyroid function tests, ultrasonography (USG), fine needle aspiration cytology (FNAC), computed tomography (CT scan), histopathology etc. The present study was undertaken to evaluate the role of ultrasonography and fine needle aspiration cytology in the diagnosis of thyroid swellings. The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of both ultrasonography and fine needle aspiration cytology were evaluated and correlated with the histopathology diagnosis.Materials & Methodology: 70 consecutive patients presenting with thyroid swellings were thoroughly examined in the department of otorhinolaryngology and details were documented. The patients were subjected to an USG and FNAC of the thyroid swellings to study the lesions from sonological and cytological point of view respectively. Patients were posted for thyroidectomy surgery and the thyroid specimen was subjected to histopathological examination. Data were collected and tabulated.Results: A total of 70 patients were enrolled in the present study. 9 (12.86%) patients were males and 61 (87.14%) were females. Male to female ratio was 1:6.78. The most common age group noted was 31-40 years with 34 patients (48.57%). Histopathology was considered as the gold standard for diagnosis. Correlation of USG and FNAC diagnosis of thyroid swellings was made with histopathology by evaluating the sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic accuracy. USG showed a sensitivity of 84.61%, specificity of 91.22%, positive predictive value of 68.75%, negative predictive value of 96.29% and accuracy 90% of in thyroid swellings. On the contrary, FNAC showed a sensitivity of 92.85%, specificity of 94.64%, positive predictive value of 81.25%, negative predictive value of 98.14% and accuracy of 94.29%. Conclusion:USG and FNAC of thyroid swellings are simple, cost effective and yield quick results. Although histopathology is confirmatory and the gold standard for diagnosis, USG and FNAC in conjunction have reasonably good sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy for preoperative diagnosis of thyroid swellings.

Introduction Thyroid gland disorders, such as goiters or tumor masses, are the result of localized or systemic aberrations of the thyroid gland. The purpose of this research was to see how effective fine needle aspiration cytology (FNAC) was in detecting thyroid swelling in patients with thyroid swelling. It is critical to be able to differentiate between benign and malignant thyroid nodules to reduce unnecessary thyroid surgeries. It is hypothesized that FNAC is not a reliable diagnostic tool to detect thyroid nodules in patients with thyroid swelling and that there is a significant variation between the number of thyroid nodules detected by FNAC and the number of thyroid nodules that were eventually diagnosed as malignant by surgery. The significance of this research shows the effectiveness of diagnostic tests for thyroid nodules in patients with thyroid swelling can help reduce unnecessary surgeries and improve patient care. Methods This cross-sectional study was carried out at the Department of Pathology at a tertiary care hospital in central India, from March 1, 2022 to June 31, 2022. Fifty patients with thyroid swelling were covered in the study. All patients were chosen after an ultrasound revealed goiter on clinical grounds. Results The majority of patients in the age group 31-40 years (33.33%) were female (74.67%). About 54.67% of the patients had a single thyroid nodule. FNAC had a sensitivity of 95.38%, a specificity of 53.33%, a positive predictive value of 86.67% and a negative predictive value of 88.57%. Conclusion FNAC is a simple, economical, and commonly used first-line diagnostic method for thyroid cancer. A false negative or false positive cytological diagnosis may be produced as a result of thyroid cytology. The study emphasizes the need to improve basic healthcare in rural India by treating FNAC as a first-line diagnostic test for thyroid swellings to guide management, although it does not replace histopathology.

BackgroundFine‐needle aspiration cytology (FNAC) is a reliable method for preoperative evaluation of thyroid nodules particularly if ultrasound‐guided (USG‐FNAC). The main purpose of this study is to evaluate the efficacy of USG‐FNAC and its accuracy.MethodsWe retrospectively studied 212 thyroidectomy cases with preoperative ultrasonography and FNAC data during the period 2015–2022 using TI‐RADS for final ultrasound diagnosis and Bethesda system for cytological diagnosis.ResultsThe studied cases were 200 females and 12 males. Thyroid cancer was more prevalent under 20 years old (78.5%). Papillary thyroid carcinoma comprises 84% of all cancer cases. Significant ultrasound features (p‐value <0.05) favour malignancy were hypoechogenicity (66%), mixed echogenicity (84%), irregular border (61%), microcalcification (68%) and rim halo (63.6%). Malignancy was found in 21% of TI‐RADS‐2, 65% of TI‐RADS‐4 and 100% of TI‐RADS‐5. There is a significant difference between different categories of Bethesda system. All cases in Cat‐VI were malignant (100%). Malignancy was also found in 81% of Cat‐V, 20% of Cat‐IV, 33% of Cat‐III, 16% of Cat‐II and 43% of Cat‐I. Cytological features consistent with malignancy were as follows: grooving (94%), nuclear irregularities (89%), nuclear pseudoinclusion (89%) and little colloid (82%). In our study, USG‐FNAC sensitivity was 83%, specificity 85%, PPV 85%, NPV 83% and accuracy 84%.ConclusionUltrasound features in favour of malignancy in thyroid nodules are hypoechoic or complex echogenicity, irregular border, punctuate calcification and presence of rim halo. Cytological features in favour of malignancy are grooving, nuclear irregularities, nuclear pseudoinclusion and little or absent colloid.
