A protocol for managing urethral obstruction in male cats without urethral catheterization

Abstract: Objective To determine efficacy of a protocol for managing urethral obstruction (UO) in male cats without urethral catheterization. Design Clinical trial. Animals 15 male cats with UO in which conventional treatment had been declined. Procedures Laboratory testing and abdominal radiography were performed, and cats with severe metabolic derangements or urinary calculi were excluded. “…We tend to condemn 'Dr Google', but in the right circumstance it can be really useful. A quick browse of Google Scholar takes you to the article ‘A protocol for managing urethral obstruction in male cats without urethral catheterization’, where 11 out of 15 cats were successfully treated for urethral obstruction without catheterisation at Ohio State veterinary college at a cost of $350, rather than the $1200 to $1800 a standard workup and treatment regime would cost (Cooper and others 2010). …”

“…If delay of successful catheterization is anticipated, careful cystocentesis may decrease trauma to the urinary bladder and ease back pressure on the urethra, perhaps facilitating retrograde catheter placement [1]. In dogs, a small gauge catheter may be able to be passed around a urethral obstruction to empty the bladder of urine.…”

“…If the obstruction is not able to be overcome, emergent definitive surgery, such as a permanent urethrostomy, may be required. Medical management for FLUTD is discussed elsewhere in detail and is beyond the scope of this chapter; however, it is important to note that some FLUTD patients respond well to sedation, subcutaneous fluid therapy, maintenance of a calm environment, and administration of medications that may decrease urethral spasm [1].…”
