One of the main reasons for reforming long-term care systems is a deficient existing service infrastructure for the elderly. This article provides an overview of why and how the Korean government expanded long-term care infrastructure through the introduction of a new compulsory insurance system, with a particular focus on the market-friendly policies used to expand the infrastructure. Then, the positive results of the expansion of the long-term care infrastructure and the challenges that have emerged are examined. Finally, it is argued that the Korean government should actively implement a range of practical policies and interventions within the new system.

Kunnan tuottaman palvelun sijasta hän voi valita palveluntuottajan yksityiseltä sektorilta ja maksaa kustannukset palvelusetelillä joko kokonaan tai kunnan ennal ta määrittämään rajaan asti. Vanhuspalvelut ja valinnanvapaus -johtavien viranomaisten näkemyksiä palvelusetelistäOlli Karsio 1 Lina Van Aerschot 1 Yhteiskuntatieteiden tiedekunta, Tampereen yliopistoPalveluseteli on yksi vanhuspalveluiden markkinoistamisen väline. Kunnat saavat päättää sen käyttöönotosta ja käytön ehdoista lain sallimissa rajoissa melko vapaasti. Tämä on johtanut siihen, että kunnissa on hyvin erilaisia ratkaisuja ja lähestymis-tapoja palvelusetelin käyttöön. Palveluseteli voidaan ottaa käyttöön vain yhdessä tai lukuisissa palveluissa, sitä voidaan tarjota kaikille tiettyyn palveluun oikeutetuille tai erilaisin perustein vain joillekin heistä. Tässä artikkelissa tarkastellaan paikallisten johtavien viranomaisten haastattelujen perusteella, millaiseksi vanhuspalvelujen järjestämisen välineeksi palveluseteli kunnissa ymmärretään sekä minkälaisin perusteluin ja tavoittein palveluseteleitä on otettu käyttöön. Tutkimuksemme perusteella kunnissa käytetään palveluseteliä asiakkaiden ja palveluntuotannon ohjailun sekä kustannussäästöjen välineenä. Paikalliset markkinat ovat olennainen tekijä, sillä ilman toimivia markkinoita palveluseteli ei tarjoa todellisia valinnan mahdol lisuuksia. Palvelusetelin mahdollistamaa valinnanvapautta pohditaan asiakkaan kannalta eri tavoin. Joissakin kunnissa huonokuntoisimpien vanhusten ei ajatella kykenevän valintoihin. Asiakkaan toimintakyvyn ja kunnon lisäksi palveluseteleitä voidaan kohdentaa maksukyvyn perusteella vain hyvätuloisille. Vaihtoehtoisesti palvelusetelistä voidaan tehdä tulosidonnainen, jolloin yksityisten palvelujen käyttö on aidosti mahdollista myös pienituloisille. JohdantoSosiaali-ja terveyspalvelujen uudistamisessa on viime vuosina korostettu valinnanvapautta. Sen lisääminen on yksi meneillään olevan SOTE-uudistuksen keskeisimpiä tavoitteita. The labor market is a complex ecosystem comprising diverse, interconnected entities, such as industries, occupations, skills, and firms. Due to the lack of a systematic method to map these heterogeneous entities together, each entity has been analyzed in isolation or only through pairwise relationships, inhibiting comprehensive understanding of the whole ecosystem. Here, we introduce Labor Space, a vector-space embedding of heterogeneous labor market entities, derived through applying a large language model with fine-tuning. Labor Space exposes the complex relational fabric of various labor market constituents, facilitating coherent integrative analysis of industries, occupations, skills, and firms, while retaining type-specific clustering. We demonstrate its unprecedented analytical capacities, including positioning heterogeneous entities on an economic axes, such as 'Manufacturing-Healthcare'. Furthermore, by allowing vector arithmetic of these entities, Labor Space enables the exploration of complex inter-unit relations, and subsequently the estimation of the ramifications of economic shocks on individual units and their ripple effect across the labor market. We posit that Labor Space provides policymakers and business leaders with a comprehensive unifying framework for labor market analysis and simulation, fostering more nuanced and effective strategic decision-making.
