A qualitative study of the impact of severe asthma and its treatment showing that treatment burden is neglected in existing asthma assessment scales

Abstract: The burden of OCS in severe asthma is neglected in policy and practice because it is not assessed in outcome studies. Existing asthma HRQoL scales provide an overly positive estimation of HRQoL in patients with frequent exposure to OCS and underestimate the benefit of interventions that reduce OCS exposure. Changes to existing measurement procedures are needed.

“…Our study confirmed that the treatment-burden themes and subthemes proposed by Eton et al24 and previously described in other chronic conditions, such as stroke,5,13 diabetes,5,14,15 asthma,16,17 and cystic fibrosis,18,19 were highly relevant for patients with COPD. The results of this study also suggest that as in other chronic conditions, COPD patients weigh the perceived burdens of treatment against their benefits.…”

“…The treatment burden for some other chronic conditions, such as stroke,5,13 diabetes,5,14,15 asthma,16,17 and cystic fibrosis,18,19 has been studied, and important burdens have been reported, including poor communication with health care providers, medication burden, time burden, and emotional distress. It is likely that the treatment burden is similarly important, especially in severe COPD, given the high number of demanding treatments.…”

“…Regular use of OCS in asthma is however associated with greater risks of systemic corticosteroid-related complications [14]. The short- and long-term detrimental adverse effects includes osteoporosis, diabetes and heart failure [1517] and the consequences appear to be related to the exposure time and OCS dosage [18].…”

“…Most validated measures of treatment burden have been developed for specific chronic conditions, such as diabetes and asthma [12,13]. Even within some of these studies, treatment burden has generally been included as one component within a multidimensional instrument, specifically designed to measure overall quality of life and treatment satisfaction [10].…”
