Urinary incontinence related to perineal muscle strength in the first trimester of pregnancy: cross-sectional study RESUMENObjetivo: Analizar la fuerza de los múscu-los del suelo pélvico (FMSP), la continencia urinaria y la calidad de vida en mujeres en el primer trimestre del embarazo. Método: Estudio transversal cuyo muestra incluyó 500 mujeres que comenzaron la atención prenatal en un servicio del sector de salud complementaria en Guarulhos, SP, en 2012-2013. La FMSP fue evaluada por la perineometría y las mujeres embarazadas con incontinencia urinaria (IU) respondieron al International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF). Resultados: Muestran que edad materna (OR=1,06 IC95% 1,02-1,11) y IU previa (OR=15,12; IC95% 8,(19)(20)(21)(22)(23)(24)(25)(26)(27)92) son las variables que, en conjunto, mejor explican la ocurrencia de IU al inicio del embarazo. La media del escore ICIQ-SF fue 8,2 (d.p.=3,9), considerado como de impacto moderado en la calidad de vida. Conclusión: Las embarazadas con más edad y con IU previa tienen chance mayor de presentar IU en el primer trimestre del embarazo. DESCRIPTORES ABSTRACTObjective: To analyze pelvic floor muscle strength (PFMS), urinary continence and quality of life related to urinary incontinence (UI) of women in the first trimester of pregnancy. Method: Cross-sectional study with a sample of 500 women who started prenatal care in a complementary healthcare facility in Guarulhos, state of São Paulo, from 2012 and 2013. Pelvic floor muscle strength was evaluated through perineometry. The pregnant women who presented UI answered the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF). Results: It was found that maternal age (OR=1.06; CI95% 1.02-1.11) and prior UI (OR=15.12; 95%CI 8.19-27.92) are the variables that, in tandem, best explain the occurrence of UI at the beginning of pregnancy. The mean score on the ICIQ-SF was 8.2 (SD=3.9), considered a moderate impact on quality of life. Conclusion: Older pregnant women with prior UI are more likely to have UI in the first trimester of pregnancy. DESCRIPTORS

BackgroundThe aim of this study was to estimate the incidence of second-degree perineal tears, obstetric anal sphincter injuries, and high vaginal tears in primiparous women, and to examine how sociodemographic and pregnancy characteristics, hereditary factors, obstetric management and the delivery process are associated with the incidence of these tears.MethodsAll nulliparous women registering at the maternity health care in Region Örebro County, Sweden, in early pregnancy between 1 October 2014 and 1 October 2017 were invited to participate in a prospective cohort study. Data on maternal and obstetric characteristics were extracted from questionnaires completed in early and late pregnancy, from a study-specific delivery protocol, and from the obstetric record system. These data were analyzed using unadjusted and adjusted multinomial and logistic regression models.ResultsA total of 644 women were included in the study sample. Fetal weight exceeding 4000 g and vacuum extraction were found to be independent risk factors for both second-degree perineal tears (adjusted odds ratio 2.26 (95% CI: 1.22, 4.19) and 3.28 (95% CI: 1.73, 6.21) respectively) and OASI (adjusted odds ratio 5.93 (95% CI: 2.29, 15.33) and 4.31 (95% CI: 1.49, 12.5) respectively), whereas age above 25 years only increased the risk of second-degree perineal tears (adjusted odds ratio 1.94 (95% CI: 1.10, 3.43)). Heredity of pelvic floor dysfunction and/or connective tissue deficiency, induced labor, and fetal head circumference exceeding 35 cm were independent risk factors for high vaginal tears (adjusted odds ratio 2.33 (95% CI: 1.09, 5.00), 3.64 (95% CI: 1.54, 8.59) and 2.86 (95% CI: 1.43, 5.75) respectively).ConclusionsThis prospective study showed that fetal weight exceeding 4000 g and vacuum extraction are independent risk factors for both second-degree perineal tears and OASI in primiparous women. Risk factors for high vaginal tears were heredity of pelvic floor dysfunction and/or connective tissue deficiency, induced labor, and fetal head circumference > 35 cm. These tears have been found to be common among primiparous women, and merit more attention.Brief summary: This prospective cohort study showed that fetal birthweight exceeding 4000 g and vacuum extraction increased the risk of second-degree tear and anal sphincter injury.

Introdução: O parto pode influenciar a força muscular do assoalho pélvico (FMAP), com possíveis morbidades do trato gênito-urinário e anal de forma transitória ou permanente. Objetivos: 1. Investigar a prevalência de infecção do trato urinário, incontinência urinária, incontinência anal e dispareunia, em primíparas com 50 a 70 (2 meses) e 170 a 190 dias (6 meses) após o parto. 2. Analisar a variação FMAP, mediante perineometria em primíparas de acordo com o tipo de parto, idade materna, escolaridade, cor da pele, situação conjugal, ocupação, índice de massa corpórea (IMC), infecção do trato urinário (ITU), incontinência urinária (IU) e anal (IA), exercícios perineais, dispareunia, intervenções e condições do períneo no parto e do recém-nascido (RN), com 50 a 70 e 170 a 190 dias após o parto. Método: Coorte prospectiva com 99 primíparas recrutadas em maternidade pública de Itapecerica da Serra, São Paulo. Colheramse os dados em três etapas: a 1ª, na internação hospitalar, até o momento da alta; a 2ª e 3ª, 50-70 dias e 170-190 dias após o parto, respectivamente, nas quais foi mensurada a FMAP. O trabalho foi aprovado pelo Comitê de Ética e Pesquisa da Escola de Enfermagem da Universidade de São Paulo (CAAE:13545113.5.0000.5392). Resultados: Considerando o período anterior à gestação houve um aumento de 13,1 pontos percentuais (p.p.) na prevalência de IU na gestação e uma redução de 16,1 (p.p.) e de 21,2 (p.p.) aos dois e aos seis meses pós-parto, respectivamente. A prevalência de IA, foi de 6,1%, aos dois meses após o parto e aos seis meses, somente uma mulher persistiu com incontinência de flatos. Aos dois e seis meses pós-parto, a prevalência de dispareunia foi referida por 44,3% e 9,5% das mulheres, respectivamente. Apesar da maior média da FMAP das mulheres após o parto normal comparada à cesariana (22,0 e 21,0 cmH 2 O, na etapa 2 versus 26,8 e 24,4 cmH 2 O, na etapa3, respectivamente), não houve diferença estatística (p=0,508). A análise bivariada apontou diferença estatística nas médias da FMAP em relação à idade (p=0,001), e o exercício perineal ficou próximo da significância (p=0,054). Não foram observadas interações entre idade e exercícios com as etapas 2 e 3. Também ocorreu associação significante entre FMAP de mulheres que relataram ITU (p=0,012) e aquelas sem IU (p=0,021). Foi obtida maior FMAP entre as participantes que não receberam anestesia (p=0,028), com diferença estatística. A FMAP não diferiu quanto às variáveis cor da pele, situação conjugal, ocupação, IMC, dispareunia, intervenções no parto e condições do RN. A análise pelo modelo longitudinal preditivo mostrou associação estatística entre idade, exercício e FMAP (p=0,005), indicando diminuição da FMAP em 0,709 a cada ano de vida da mulher e aumento de 3,359 cmH 2 O na média da FMAP naquelas que realizaram exercícios perineais. Conclusão: A FMAP não difere quanto ao tipo de parto. As primíparas, com ITU e sem IU, que realizaram exercícios perineais e que não receberam anestesia local apresentaram estatísticamente maior FMAP. Não foi possível ...
