A randomized, placebo-controlled pilot trial of armodafinil for fatigue in patients with gliomas undergoing radiotherapy

Abstract: While treatment was well-tolerated, an 8-week course of armodafinil did not improve fatigue or QOL in glioma patients undergoing RT in this pilot study. Further studies are needed to determine whether pharmacologic treatment improves fatigue in glioma patients undergoing RT.

“…In our published manuscript on the primary study aim for insomnia [30], we also saw no effect of armodafinil on insomnia or sleep quality. This lack of significant effect is consistent with previous trials that have also demonstrated no effect of armodafinil on QOL in patients with cancer, even for doses as high as 400 mg/day [4043]. This might be due to the fact that it may be difficult to improve insomnia beyond the effects produced by CBT-I, as several well-conducted RCTs have indicated that CBT-I produces robust clinical improvements in cancer-related sleep problems [18].…”

“…34 Other studies report no evidence of benefit on depressive symptoms. 22,23,[35][36][37] These negative results are difficult to interpret because these studies were neither focused on, nor powered to exclude an effect on depression.…”

“…22 Several RCTs examined the effect of methylphenidate or armodafinil on fatigue in patients undergoing radiotherapy for brain metastases. 23,24 These studies failed to find a significant reduction in fatigue, although post-hoc analyses noted improvement at 4-week follow-up in those with high baseline fatigue and a trend towards improvement at 6-week follow-up using armodafanil. 23 Thus, there is no evidence suggesting a significant benefit from modafinil, armodafinil, methylphenidate or donepezil for the treatment of fatigue in stable brain tumour patients after treatment.…”
