A Randomized Study Comparing Levofloxacin, Omeprazole, Nitazoxanide, and Doxycycline versus Triple Therapy for the Eradication of Helicobacter pylori

Abstract: OBJECTIVES:Resistance to standard Helicobacter pylori (HP) treatment regimens has led to unsatisfactory cure rates in HP-infected patients. This study was designed to evaluate a novel four-drug regimen (three antibiotics and a proton pump inhibitor (PPI)) for eradication of HP infection in treatment-naive patients.METHODS:Patients with a diagnosis of HP gastritis or peptic ulcer disease confirmed using endoscopy and stool antigen testing were eligible for inclusion in this study. All patients underwent a washo… Show more

“…In an open-label, randomized study of 270 patients conducted in the United States, LOAD given for 7 or 10 days, yielded eradication rates of 89 and 90% compared with 73% with a 10-day course of lansoprazole, amoxicillin, and clarithromycin ( 106 ). No data were provided in the manuscript regarding the impact of levofl oxacin resistance on the effi cacy of LOAD therapy, and additional trials using this unconventional and expensive regimen are awaited.…”

“…It led to 88.9% eradication for 10 days of LOAD, 90% for 7 days compared to 73.3% for standard triple therapy, with a number needed to treat to achieve one more successful eradication of 6 [24]. A meta-analysis of nonbismuth containing concomitant/quadruple regimens showed a mean cure rate of 88% across more than 2000 patients [25]. Other original studies have focussed this year on various forms of quadruple and concomitant therapy.…”

“…Furthermore, the future of susceptibility testing may be noninvasive stool PCR-based testing, which will obviate direct culture (37). A future study might be enhanced by including resistance data for additional antibiotics which are sometimes used to treat refractory H. pylori, such as furazolidone, rifabutin, and nitazoxanide (38)(39)(40). In conclusion, although secondary resistance of H. pylori to CLR and MET remains high, rates do not appear to be increasing at this stage.…”
