A rare association of tuberculous longitudinally extensive transverse myelitis (LETM) with brain tuberculoma

Abstract: BackgroundLongitudinally extensive transverse myelitis is characterized by contiguous inflammatory lesion of spinal cord involving three or more spinal segments. It is a well-recognized but rare presentation of Mycobacterium tuberculosis infection.Case descriptionWe report a case of young boy diagnosed with multiple brain tuberculomas. He was on antitubercular drugs therapy for 2 months and became asymptomatic. On 2-month followup visit, the patient complained of acute onset progressive sensorimotor, spastic p… Show more

“…Two patients were follow-up case of tubercular meningitis taking anti tubercular treatment and presented to us with new spinal cord lesion in the form of LETM, and remaining one patient newly presented with LETM and proved to be having tubercular etiology. In our series, tuberculosis was found as an important but rare etiology of LETM [25,26]. In our series of tubercular LETM, one the patient had brain tuberculomas and none were associated with radiculopathy.…”

“…Previous reports have considered a combination of steroid and antitubercular therapy to be appropriate. 3 4 18 41 42 43 44 Antiretroviral therapy may be added to the treatment regime for patients with HIV. All of the reported patients analyzed in this review received a combination of steroid and antitubercular therapy, with follow-up clinical and radiological improvements evident in most of the patients.…”

“…In the case of Jain et al ., at a 2-month follow-up visit, LETM was found to accompany brain tuberculoma as a paradoxical expansion after MTB treatment. [ 5 ] Paradoxical tuberculous LETM is seen in association with brain tuberculomas when patients are placed on antitubercular drugs. The initial clinical response to treatment may cause development of new lesions or expansion of preexisting ones due to lysis and release of tubercular proteins.…”
