A rare case of Burkitt's lymphoma of the duodenal bulb

Abstract: Gastrointestinal tract involvement in immunodeficiency-related Burkitt’s lymphoma is not common and the duodenal involvement is very rare. We report the case of a 35-year-old man admitted because of abdominal pain, vomiting and weight loss. Human immunodeficiency virus infection was diagnosed and upper digestive tract endoscopy showed marked edema and hyperemia of the duodenal bulb with some violaceous areas. Immunohistochemical study of the bulbar tissue samples confirmed the diagnosis of Burkitt’s lymphoma. … Show more

“…Usually; Burkitt lymphoma is common in patients with AIDS, when treating immunocompromised patients with chronic abdominal symptoms, especially those with complications such as bleeding or occult positive testing, there should be a high level of suspicion for lymphomas. Early intervention with endoscopy and biopsy can aid in the identi cation of these conditions, leading to improved outcomes for patients [9,10]. when treating gastrointestinal lymphoma; Takagishi et al recommend a multidisciplinary approach with this regimen: rst, rituximab alone, followed by less intensive chemotherapy without vincristine, when the patient's health improves and the mass size decreases, complete chemotherapy may be administered.…”

“…1), consistent with the published literature. [23][24][25] Although we found endoscopic abnormalities in all patients, it is important to note that small bowel lymphomas may also present with normal mucosa 26,27 and may mimic other conditions such as Crohn disease, 28 therefore requiring a high index of suspicion. Endoscopic treatment of complications has also been occasionally described, for example endoscopic ultrasound-guided gastrojejunostomy and choledocoduodenostomy for malignant intestinal and biliary obstruction, 29 endoscopic balloon dilation for duodenal stenosis, 30 or placement of covered self-expanding metal stent for duodenal-ileal fistula healing.…”

“…The immunodeficiency related Burkitt lymphoma usually is held accountable for over 30% of lymphomas in immunocompromised patients but very few cases have ever been reported specifically in the duodenal region of the gastrointestinal tract. Burkitt lymphoma is known to be an aggressive B-cell lymphocytic malignant tumor that has commonly presented in the gastrointestinal tract mainly in stomach but rarely in the duodenum [ 5 – 7 ].…”

“…In conclusion, a high suspicion of lymphomas should be present while treating immunocompromised patients with chronic abdominal symptoms especially with complications such as bleeding or occult positive testing. Early endoscopy intervention with biopsy can help identify these conditions and improve outcomes for the patients [ 2 , 5 , 7 ]. Burkitt lymphoma is a malignancy that has commonly presented in gastrointestinal tract but rarely in the duodenum.…”
