A rare case of Raoultella planticola urinary tract infection in an immunocompromised patient with multiple myeloma

Abstract: IntroductionRaoultella planticola is a gram-negative rod associated with soil and aquatic environments that has rarely been associated with human infections.Presentation of caseWe present the case of a 73 year old female with multiple myeloma and recurrent Clostridium difficile infection who was found to have a urinary tract infection with Raoultella planticola, which has only been reported to cause 29 cases of human infections and 2 cases of urinary tract infections.Discussion and conclusionOur case and liter… Show more

“…To date, 29 cases of human infection with R planticola has been reported with only 3 urinary tract infections. 3 , 4 R planticola is difficult to isolate and to identify, as it can easily be confused with other genera, especially klebsiella. 5 R planticola rarely cause infection in healthy individuals.…”

“…Mortality is high in immunocompromised patients. 3 The First reported human infection due to Klebsiella trevisanii (later reclassified as R planticola ) was in 1986, which included bacteremia in a 69-year-old patient. 10 In 2014, a case of R Planticola bacteremia in a 56-year-old female was reported following consumption of seafood salad containing squid and octopus.…”

“…Antimicrobial resistance (AMR) of Raoultella causing human infections has not been analysed systematically. However, Raoultella species harbouring extended spectrum β-lactamase (ESBL) and carbapenemase genes have been reported [14][15][16][17]. Most of the cases were reported in Europe and the USA.…”

“…It is a histamine-producing bacterium generally found in aquatic environments, in fish and in soil (Lam and Salit, 2017). It is not generally thought to be pathogenic, but recent cases of human infection are emerging, most commonly in fragile, frequently hospitalized patients with multiple comorbidities and in the pediatric population (Kalaria et al, 2017;Mehmood et al, 2018;Serkan et al, 2018;Skelton et al, 2017;Xu et al, 2016). There is one concerning case report of carbapenem resistance with R. planticola infection (Skelton et al, 2017).…”
