A retrospective analysis of the prognosis of prostate cancer patients with lymph node involvement on MR lymphography: who might be cured

Abstract: BackgroundThe prognosis of prostate cancer patients with lymph node metastases so small they can only be visualized by new imaging techniques as MR lymphography (MRL) is unknown. The purpose of this study was to investigate the prognosis of prostate cancer patients with non-enlarged metastatic lymph nodes on MRL and to identify a subgroup of MRL-positive patients who might be candidates for curative treatment.MethodsThe charts of 138 prostate cancer patients without enlarged lymph nodes on CT, in whom a pre-tr… Show more

“…With CEM as a technique to (re)define oligometastatic disease, these therapies might even be given with curative intent. 4,5,51,52 Furthermore, knowledge on size of lymph node metastases can help predict prognosis. Patients with nodal metastases ≤8 mm versus larger nodal metastases have a significantly better 5-year metastasis-free survival (79% vs 16%) and overall survival (81% vs 36%).…”

“…Meijer et al[ 31 ] showed a better prognosis in a subset of patients with MRL positive lymph nodes that were < 8 mm and better outcomes in patients in whom all MRL positive lymph nodes were resected. This highlights a window of opportunity for cure in these patients as those with MRL-detected positive nodes that were entirely removed had a five-year distant metastatic disease free survival of 80% compared to 35% in the patients who did not have all MRL-detected positive nodes removed[ 31 ].…”

“…Many radiological and less invasive methods (including sentinel node biopsy) are described to determine lymph node involvement in prostate cancer. [ 6 7 ] However, till date, pelvic lymph node dissection is the only reliable and effective method to ascertain the presence of lymph node metastases in patients undergoing radical prostatectomy. Various nomograms, based on pre-operative parameters, have been described to predict the probability of lymph node positivity.…”
